电子束CT增强双期扫描和血管重建对Budd-Chiari综合征的评价

摘    要:目的 分析Budd-Chiari综合征的CT增强双期扫描影像学征象,评价血管三维重建的临床意义。方法 32名患者CT扫描增强动脉期延迟时间为30s,门脉期延迟时间为70s。血管三维重建方式为表面阴影遮盖法(SSD)和多平面重建法(MPR)。所有患者均进行了下腔静脉数字减影血管造影。结果 下腔静脉梗阻:隔膜性5例,闭塞性15例,狭窄性12例。肝静脉完全梗阻10例,不完全梗阻14例。可发现有两种肝外侧支循环形成,其中10例有肝内侧支循环形成。SSD法血管重建可显示血管和侧支循环总体情况,而MPR法可多角度直接显示血管。结论 电子束CT增强双期扫描和血管三维重建可全面评价Budd—Chiari综合征的血管和器官病变状况。

Abstract:Objective To analyze the images of the Budd Chiari syndrome by dual phase CT and evaluate the clinic value of vessel reconstruction. Methods CT scans were performed on 32 patients. The hepatic arterial phase began at the 30th second and the portal venous phase at the 70th second after injection. Surface shaded display (SSD) and Multi planar reformatting (MPR) were applied in vessel reconstruction. Digital subtraction angiography of inferior vena cava (IVC) was performed on all patients. Results The types of obstruction of IVC include membranous obstruction (5 cases),occlusion (15 cases) and stenosis (12 cases). There were two kinds of extrahepatic collateral venous pathways,and intrahepatic collateral venous pathways can be seen in ten of them. The information of vessels and collateral pathways could be showed with SSD,and MPR could directly display vessels by multi angles. Conclusion Dual phase electron beam CT and vessel reconstruction can evaluate the condition of vessels and tissues in the Budd Chiari syndrome.
