强直性脊柱炎早期及活动期MRI表现

[目的]探讨强直性脊柱炎(AS)早期及活动期的MRI表现,以提高其诊断水平。[方法]回顾性分析AS患者的骶髂关节及脊柱26例MRI的表现特点。[结果]本组12例病人均有双侧骶髂关节滑膜软骨MRI信号改变,其中5例还有骨髓信号改变。13例病人23个骨突关节出现关节突骨髓及周围软组织水肿,其中3例行增强扫描。6例病人17个椎体可见椎缘炎。2例前纵韧带水肿。3例棘间韧带水肿。2例脊柱呈后弓驼背畸形,并可见伴脊肌萎缩。[结论]MRI检查可对未确诊早期AS的诊断能提供重要的影像信息;对已确诊AS的临床活动性期判断及病情分期评估有重要帮助。
