内镜下气囊扩张术治疗贲门失弛缓症的中远期疗效

摘    要:目的:观察内镜下气囊扩张术治疗贲门失弛缓症的中远期疗效,探索贲门失弛缓症的非手术治疗方法。方法:对内镜/钡餐检查诊断为贲门失弛缓症的68例患者,用直径3.0cm,3.5cm气囊行内镜下气囊扩张治疗,疗效判断按Stooler分级法,吞咽困难减轻1级为有效,减轻2级或无醒阻为显效,以首次扩张治疗后6月-1年为中期疗效,1年以上为远期疗效,计算该疗法的中远期有效率,结果:中期疗效达100%,远期疗效为98.4%,少数复发病例再次扩张治疗后吞咽困难症状缓减,无食道大出血,穿孔等重并发症发生,结论:内镜下气囊扩张术治疗贲门失弛缓症具有良好的中远期疗效,值得推广应用。

Abstract:Objective:To observe the medium-long-term curative effect of endoscopic balloon-dilating therapy for achalasia.To explore a kind of non-surgical method for achalasia.Methods:68 cases with achalasia diagnosed by endoscopy/x-ray were treated with endoscopic balloon-dilating therapy,balloons of 3.0cm and 3.5cm in diameters were used,curative effect was evaluated according to Stooler method: dysphagia relieve one scale was considered good,dysphagia relieve two scales or no-dysphagia was considered excelled 6 months to 1 year after first dilation was medium-term effect,more than 1 year was long-term effect,medium-long-term effective rate was calculated.Results:Medium-term effective rate was 100%,long-term effective rate was 98.4%,a few cases with recurrent dysphagia were retreated with dilation and dysphagia was relieved.No serious complications such as massive hemorrhage and perforation of esophagus occurred.Conclusions:Endoscopic balloon-dilating therapy for achalasia had good medium-long-term curative effect and deserved for further application.
