P73和增殖细胞核抗原在胃癌中表达的意义

目的:检测P73、增殖细胞核抗原(PCNA)在胃癌中的蛋白表达,探讨其临床病理意义。方法:选取40例胃癌及20例萎缩性胃炎作为对象,应用免疫组织化学方法(SP法)检测P73、PCNA的表达情况,并结合临床病理资料进行统计学分析。结果:①P73蛋白在萎缩性胃炎组中表达率为20.0%(4/20),在胃癌组中的表达率为62.5%(25/40),二者比较有显著性差异(P<0.01);PCNA蛋白在萎缩性胃炎组中表达率为55.0%(11/20),在胃癌组中的表达率为80.0%(32/40),二者比较有显著性差异(P<0.05)。②胃癌组中P73蛋白的阳性表达率与胃癌组织类型、浸润深度及淋巴结转移无关(P>0.05);胃癌组中PCNA蛋白的强阳性表达率与胃癌组织类型、浸润深度、淋巴结转移密切相关(P<0.05)。③胃癌组中P73蛋白与PCNA蛋白表达成正相关(rs=0.387,P<0.05)。结论:P73作为抑癌基因有其组织特异性,P73、PCNA的表达与胃癌临床病理和生物学行为存在密切关系,是胃癌细胞增生活跃的重要影响因素。

Expression of P73 and PCNA and Its Significance in Gastric Carcinoma

Objective:To investigate the expression of protein P73 and PCNA(prolife rating cell nuclear antigen) in gastric carcinoma and its clinical pathological significances.Methods:Immunohistochemical staining(SP method) was used to detect the expression of P73 and PCNA protein in 40 cases of gastric carcinoma and 20 cases of chronic atrophic gastritis.The correlation of P73 and PCNA expression with clinicopathological data was statistically analyzed. Results:① The positive rate of P73 protein expression in chronic atrophic gastritis group and gastric carcinoma group were 20.0%(4/20) and 62.5%(25/40) respectively with significant difference(P<0.01).The positive rate of PCNA protein expression was significantly different between chronic atrophic gastritis group(55.0%,11/20) and gastric carcinoma group(80.0%,32/40)(P<0.05).② The positive rate of P73 protein expression in gastric carcinoma group had no correlation with the histological type,depth of invasion of gastric mucosa and lymph node metastases(P>0.05).The strong expression for PCNA in gastric carcinoma group was correlated with the histological type,depth of invasion of gastric muscosa and lymph node metastases(P<0.05).③ The expression of P73 was positively correlated with the expression of PCNA in gastric carcinoma group(rs=0.387,P<0.05).Conclusion:As a tumor suppressor gene,P73 has its tissue specificity,and the expression of P73 and PCNA is significantly related to pathological characteristics of gastric carcinoma and their biological behaviors.Both P73 and PCNA play important roles in the distinct hyperplasia of gastric cancer.
