以耐信、呋喃唑酮为核心的四联疗法治疗一线、二线根治失败的幽门螺杆菌感染

目的观察以耐信、呋喃唑酮为核心的四联疗法治疗一线、二线根治失败的幽门螺旋杆菌感染的疗效。方法行胃镜检查以胃黏膜快速尿素酶检测、病理HE染色检查方法确认为幽门螺旋杆菌感染的患者，以国际标准一线或二线治疗方案进行根治，停药4周后13^C呼气试验仍为阳性的患者判定为根治失败，用本研究方案再次治疗并以13^C呼气试验检测判定治疗结局．同时检测患者血清CagA抗体。结果22例一线或二线治疗幽门螺旋杆菌感染失败的患者，以耐信、呋喃唑酮为核心的四联疗法再次治疗1周后，19例Helicobacter pylori根治成功，成功率为86．4％，副作用发生率为22．7％，主要为恶心，症状轻微，治疗费用低。结论以耐信、呋喃唑酮为核心的短程四联疗法对一线、二线根治失败的幽门螺旋杆菌感染是一种安全、经济、有效的治疗方案选择。

The quadruple regimen including Nexium and furazolidone on a choice to treat helicobacter pylori eradication failure

Objective To investigate the efficacy of Nexium-and-furazolidone- based quadruple therapy to eradicate helicobacter pylori after first-line or second-line treatment failure. Methods Patients with helicobacter pylori infection confirmed by rapid urease test and pathological exam received first - line or second - line international therapy were enrolled. The initial treatment failed if 13^C breath test was positive 4 weeks later. Then such patients received Nexium-and-furazolidone-based quadruple therapy. 13^C breath test was used again to see whetherthe eradication of helicobaeter pylori was finally achieved or not. Results Twenty-two patients with initial treatment failure were included in our study and 19 patients achieved eradication of heicobacter pylori after one-week Nexium-and-furazolidone-based quadruple therapy. The eradication rate was 86.4% and minor adverse affects including nausea happened in 22.7% of patients. The associated symptoms disappeared after the quadruple therapy. The medical fee was low as well,Conclusion Nexium-and-furazolidone-based quadruple therapy is an effective, safe, economical choice after first-line or second - line international treatment failure for helicobacter pylori infection.
