老年2型糖尿病合并高尿酸血症的临床分析

目的探讨老年2型糖尿病高尿酸血症的临床特征。方法将1411例老年体检干部分为糖尿病高尿酸血症组、糖尿病正常尿酸血症组和非糖尿病对照组，测定其体重指数（BMI）、腰臀比（WHR）、血压（SBP、DBP）、血糖（FBG、PBG）、甘油三酯（TG）、胆固醇（CHO）、高密度脂蛋白（HDL-C）、低密度脂蛋白（LDL-C）、空腹血清胰岛素（FINS）、血肌酐（Cr）、血尿酸（UA）。结果与老年糖尿病正常尿酸血症组相比，老年糖尿病高尿酸血症组的BMI、WHR、DBP、TG、FINS、HOMA-IR、Cr水平明显升高，合并代谢综合征、肥胖、高甘油三酯血症、低HDL-C血症和胰岛素抵抗的患病比例明显增高，而HDL-C水平降低。Logistic回归分析显示，DBP、Cr、TG、HDL-C、FINS、HOMA-IR是老年糖尿病高尿酸血症的危险因素。结论舒张压、甘油三酯、低密度脂蛋白、胰岛素抵抗、肌酐与老年2型糖尿病高尿酸血症密切相关。

Analysis of correlation factors of hyperuricemia with type 2 diabetes mellitus in elderly patients

Objective To explore the correlation factors of hyperuricemia with type 2 diabetes mellitus in elderly patients.Methods 1411 elderly cadres were included.Blood glucose(FBG,PBG),creatinin(Cr),cholesterol(CHO),triglyceride(TG),blood pressure(SBP,DBP),body mass indes(BMI),waist hip ratio(WHR),fasting serum insulin(FINS) and uric acid(UA) were measured.Results Eldly patients with hyperuricemia in type 2 diabetes had higher BMI,WHR,DBP,TG,FINS,HOMA-IR and Cr than those with normal serum uric acid.The hyperuricemia group had higher proportion of metablic syndrome and its related diseases,such as diabetes mellitus,obesity and insulin resistance than the normal-uricemia group.Logistic regression analysis results suggested that DBP,TG,FINS,HOMA-IR and Cr were independent risk factors of hyperuricemia in eldly type 2 diabetes.Conclusions DBP,TG,FINS,HOMA-IR and Cr were more related to hyperuricemia in eldly type 2 diabetes.
