CT引导下经皮肺穿刺活检并发症的影响因素分析

目的探讨CT引导下经皮肺穿刺活检并发症的预防和处理,并分析其影响因素。方法分析南方医科大学珠江医院346例CT引导下经皮肺穿刺活检患者并发症种类、影响因素及其处理措施。结果除6例需行二次穿刺外,其余患者均一次穿刺成功。并发症主要为气胸（45例）及肺出血（31例）,气胸主要与病灶部位及穿刺针行径有关,肺出血主要与病灶部位及大小相关。少量气胸或出血无需特殊处理,大量气胸或出血较多时需及时行胸膜腔穿刺术,必要时留置胸膜腔闭式引流管。结论 CT引导下经皮肺穿刺是一种安全、有效的诊断方法,具有协助明确诊断及指导进一步治疗的价值。

Analysis on Influencing Factors of the Complications Induced by CT-guided Lung Biopsy

Objective To explore the prevention and treatment of the complications induced by CT-guided lung biopsy,and analyze their influencing factors.Methods Three hundred and forty-six cases of CT-guided lung biopsy with percutaneous needle hospitalized in Zhujiang Hospital of Southern Medical University were retrospectively analyzed,and the attention was focused on the types and risk factors of the complications and the prevention and treatment measures.Results Three hundred and forty cases were successfully punctured by the first time,while the other 6 were treated on the second chance.The main complications were pneumothorax(45 cases) and pulmonary hemorrhage(31 cases).In our analysis,pneumothorax tended to closely relate to the lesion site and pricking way,while pulmonary hemorrhage tended to closely relate to lesion size and site.Slight pneumothorax or bleeding did not need other treatment,while severe one required immediately exhaust aspiration or chest tube drainage.Conclusions CT-guided lung biopsy was proved to be an effective and safe method,which could provide important information for clinical diagnosis and feasible treatment strategy.
