血清氨基末端脑钠肽前体水平与冠心病高危因素的相关性研究

摘    要:目的对长沙市某单位职工血清氨基末端脑钠肽前体(NT-proBNP)水平进行监测,并初步探讨其与冠心病高危因素之间的关系。方法采用流行病学方法,按设计表格对参检人员进行个人因素调查,检测体重指数(BMI)、血压、空腹静脉血糖(GLU)、胆固醇(CHOL)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)和尿酸(UA)等生化指标,同时采用荧光酶免疫法检测血清NT-proBNP水平。结果 1 508例某单位职工血清NT-proBNP水平为(73.17±63.39)pg/ml,其中高危组(98.96±83.73)pg/ml明显高于正常组(52.64±26.14)pg/ml(t=15.124,P=0.000)。在正常组中,男性血清NT-proBNP水平高于女性(t=-8.563,P=0.000),但不同年龄段间差异无统计学意义(F=0.977,P=0.403)。在高危组中,性别对NT-proBNP水平无明显影响(t=1.778,P=0.070),但不同年龄段间差异有统计学意义(F=2.965,P=0.031),以60~70岁组最高;不同收缩压、舒张压、体重指数和低密度脂蛋白组间的血清NT-proBNP水平差异均有统计学意义(P〈0.05),而不同甘油三酯、总胆固醇、高密度脂蛋白、尿酸、糖尿病和家族冠心病史组间差异无统计学意义(P〉0.05);随着危险因素个数的增加,血清NT-proBNP水平也明显增加,组间差异有统计学意义(F=12.487,P=0.000)。结论血清NT-proBNP水平与冠心病危险因素有关。

Abstract:Objective To monitor the serum level of NT-proBNP among the employees of a certain unit in Changsha,and to explore its relationship with risk factors of coronary heart disease.Methods With the epidemiological method,the individual information of the employees was collected according to the designed table.Body mass index(BMI) and blood pressure were measured.Biochemical indexes,including blood sugar(GLU),cholesterol(CHO),triglyceride(TG),high density lipoprotein(HDL),low density lipoprotein(LDL) and uric acid(UA) were determined.The fluorescent enzyme immunoassay was used to detect the serum level of NT-proBNP.Results The average serum level of NT-proBNP in 1,508 employees was(73.17±63.39) pg/ml,and a significant difference was found between risk factor group(98.96±83.73) pg/ml and normal control group(52.64±26.14) pg/ml,(t=15.124,P=0.000).In normal control group,the serum level of NT-proBNP in male was higher than that in female(t=-8.563,P=0.000),but no statistically significant difference was found in the serum level of NT-proBNP among the different age groups(F=0.977,P=0.403).In risk factor group,there was no correlation between NT-proBNP and sex(t=1.778,P=0.070),the level of NT-proBNP in 60~70 year-old group was the highest,and the difference was statistically significant among the different age groups(F=2.965,P=0.031).In risk factor group,there were statistically significant differences in the serum level of NT-proBNP among the employees with different SBP,DBP,BMI and LDL(P<0.05),but no statistically significant difference was found in the serum level of NT-proBNP among the employees with different TG,CHO,HDL,UA,GLU and CHD history(P>0.05).With the increase of the number of risk factors,the level of NT-proBNP in risk factor group was significantly increased(F=12.487,P=0.000).Conclusions There may be a correlation between the serum level of NT-proBNP and risk factors of coronary heart disease.
