子宫肌瘤中细胞增殖和凋亡的研究

摘    要:目的通过检测子宫肌瘤组织及正常子宫平滑肌组织中增殖细胞核抗原(PCNA)和凋亡调控基因bcl-2的表达,探讨细胞增殖和细胞凋亡在子宫肌瘤发病机制中的作用.方法用免疫组织化学ABC法检测40例(增生期22例,分泌期18例)子宫肌瘤组织和正常子宫平滑肌组织中的PCNA和bcl-2蛋白的表达.结果 PCNA和bcl-2在肌瘤组织中表达均强于正常子宫平滑肌组织,且分泌期强于增生期(P<0.01).但在正常子宫平滑肌组织中两者增生期和分泌期的表达则无显著性差异(P>0.05).结论 PCNA和bcl-2表达的增加可能在子宫肌瘤的肿瘤发生过程中起重要作用.子宫肌瘤的发生不仅与肌瘤细胞增殖活性增高有关,而且与肌瘤细胞的凋亡受抑制有关.这种作用可能受到孕激素的调节.

Abstract:Through detecting the expression of proliferating cell nuclear antigen (PCNA) and bcl-2 protein in uterine leiomyomas and in normal myometrium, to explore the role of cell proliferation and apoptosis in the development of uterine leiomyoma. The expressions of PCNA and bcl-2 protein were examined by immunohistochemical ABC staining in 40 cases of uterine leiomyomas and normal myometrium (22 were from proliferative phase and 18 from the secretory phase of the menstrual cycle). PCNA and bcl-2 expression were both higher in uterine leiomyomas than those in normal myometrium both in the proliferative phase and in the secretory phase of the menstrual cycle (P <0.01); the expression of PCNA and bcl-2 in leiomyomas in the secretory phase were both stronger than those in the proliferative phase of the menstrual cycle (P <0.01). There were no apparent differences in the expression of PCNA and bcl-2 in normal myometrial smooth muscle cells between the proliferative phase and the secretory phase of the menstrual cycle (P >0.05). Conclusions] The increase expression of PCNA and bcl-2 may play an important role in the development of uterine leiomyomas. The growth of leiomyoma may not only through stimulating cell proliferation in leiomyoma cells, but also through the induction of bcl-2 protein to inhabit apoptosis in leiomyomas cells. This action may be regulated by progesterone.
