小儿肾穿刺活检术的临床意义（附19例分析）

目的：探讨小儿肾穿刺活检术在小儿肾小球疾病诊断、治疗及预后判断方面的临床价值。方法：在B超引导下，对19例符合肾穿刺活检适应症的肾小球肾炎患儿行肾穿刺活检术20例次(采用福州军区总医院自行研制的自动同步负压吸引枪，改良Menghini针，单人单手操作)，术后肾活组织行光镜、电镜及免疫组化检查，并观察肾穿刺术后并发症发生。结果：20例次中成功获取肾活组织的共19例，成功率95．0％；病理类型为：系膜增生性肾小球肾炎14例，毛细血管内增生性肾炎2例，IgA肾病1例，膜性肾病1例，微小病变1例。术后共有2例出现并发症，肉眼血尿、肾包膜下血肿各1例。结论：经皮肾穿刺活检术在小儿肾脏疾病的诊断、治疗及判断预后方面有重要的临床意义，方法简单、安全有效，值得推广。

The clinical significance of children renal biopsy:Report of 19 cases

Objective To evaluate the clinical significance of children biopsy in child glomerular diseases.Methods By the lead of B ultrasonography,we adopted the automatic synchronous subtractive press pucture needle.In 19 child cases,biopsy have been taken for 20 times.Then,light microscope,electric microscope,and immune histochemistry examine were used to the renal tissues.At the same time,the complication were researched after the renal biopsy operation.Results In all 20 cases,19 live tissues of renal were successfully taken,the successful rate is 95.0%.Pathological types were mesanioproliferative glomerulonephritis(14 cases),endocapillary pnoliferative glomerulonephritis(two cases),IgA nephropathy(1 case),membranous nephropathy(one case),minimal-change nephropathy(one case).Two cases have complications,one is naked eye hematuria,the other is perirenal hematoma.Conclusion Renal biopsy has important significance on the diagnosis,treatment,and prognosis of child glomerular diseases.This is a simple,safe and valuble method.
