老年消化性溃疡129例临床与胃镜特点分析

目的探讨老年消化性溃疡的临床与胃镜特点。方法回顾性分析并比较我院内镜中心近2年来经胃镜检出的老年消化性溃疡患者129例与同期检出的中青年消化性溃疡患者105例的临床及内镜特点。结果老年组患者症状不典型。大多缺乏典型腹痛症状。老年组129例，其中胃溃疡62例，十二指肠球部溃疡53例，复合溃疡14例；中青年组105例。其中胃溃疡28例，十二指肠球部溃疡68例，复合溃疡9例。两组相比胃溃疡差异具有统计学意义（P〈0．01）。老年组胃体溃疡占37．1％，直径〉2cm的溃疡占29．0％，并发症以上消化道出血为主，占42．6％，与中青年组相比较差异有统计学意义（P〈0．05，P〈0．05，P〈0．01）。结论老年消化性溃疡与中青年组相比临床症状多不典型，伴随其他慢性疾病多，胃溃疡多发，溃疡面积较大，并发症多且较严重。

Analysis of Clinical and Endoscopic Features in 129 Elderly Patients with Peptic Ulcer

Objective To investigate the clinical and endoscopic features of elderly patients with peptic ulcer. Methods The clinical and endoscopic features of 129 elderly patients with peptic ulcer confirmed by endoscopy in our hospital in the recent two years were retrospectively analyzed and compared with 105 cases of youth with peptic ulcer detected at the corresponding period. Results There were 129 elderly peptic ulcer cases in aged group, including 62 cases of gastric ulcer, 53 cases of duodenal ulcer, 14 cases of complex ulcer, and 105 peptic ulcer cases in younger group, including 28 cases of gastric ulcer, 68 cases of duodenal ulcer, and 9 cases of complex ulcer. Gastric ulcer detection had significantly statistical difference between the two groups （P〈0.01）. Gastric body ulcer was the dominant （37.1%） in the elderly group with gastric ulcer. Upper gastrointestinal hemorrhage was the most common complications, accounting for 42.3%, with significant differences in contrast to the younger group （P 〈0.05, P 〈0.05, P 〈0.01 ）. Conclusion Clinical symptoms and signs of elderly peptic ulcer are not typical accompanied by many other chronic diseases, ulcers are larger, and complications are more and severer than younger.
