外伤性眼内炎30例分析

目的 分析外伤性眼内炎危险因素及玻璃体类手术治疗疗效。方法 回顾性分析我科从1990～2002年收治的所有外伤性眼内炎患者30例，对其性别、年龄、外伤类型、病原体、治疗方法及其预后进行分性。结果 男性23例，平均24．8岁。女性7例，平均13．1岁。学龄前儿童和未成年人以注射器外伤为主，成年人以铁丝扎伤及铁异物为主；其中18例进行病原体检查，阳性率为55％，革兰氏阳性和阴性菌分别有4例和6例。玻璃体类手术是治疗外伤性眼内炎有效的方法。结论 对不同的年龄与职业采取恰当措施可以有效预防外伤性眼内炎；对眼内炎患者应及时选择适合的玻璃体类手术治疗。

An analysis of traumatic endophthalmitis

Objective To analyze the risk factors and prognosis of traumatic endophthalmitis.Methods Retrospective analysis of all traumatic endophthalmitis in PUMC Hospital from 1990 to 2002.Criteria including age,gender,kinds of trauma,pathogen,treatment methods and prognosis were analyzed.Results There were 30 patients suffering from traumatic endophthalmitis,including 23 males and 7 females.The mean ages were respectively 24.8 and 13.1.Needle trauma became the leading cause of traumatic endophthalmitis in youngsters under 18-year old,however the iron trauma was the main cause for the adults.Vitreous samples of 18 cases were cultured,the positive rate was 55%.The pathogens include 4 cases of gram positive and 6 cases of gram negative bacteria.Vitrectomy combined with injection of antibiotics is an effective strategy(P<0.05).Conclusion Advisable measures depending on ages and professions should be done to prevent traumatic endophthalmitis.On-time vitrectomy is an effective method to treat endophthalmitis.
