| 摘 要： | 目的摸索在初治肺结核患者进行中西医结合治疗的临床疗效。方法通过结核菌染色、培养、药敏试验和流式细胞仪免疫检测技术,分析102例初治肺结核患者治疗前后的各种指标变化,并对其中3例MDR-TB患者的结核杆菌分离株进行一线和重要二线抗结核药的耐药基因突变分析。结果治疗四个月后,中医组痰菌转阴率86.4%（19/25）;西医组痰菌转阴率78.3%（18/23）。中医组临床治疗显效率94.1%（48/51）;西医组临床治疗显效率90.2%（46/51）。中医组免疫各项指标均比西医组改善明显。在3株MDR-TB分离株中可检测到大量与所测定的一线和二线抗结核药耐药相关的基因突变。结论按照中医辨证分型,有针对性的中西医结合治疗初治肺结核患者,比单纯西医组治疗效果明显。特别是在免疫调节方面效果更加明显。MDR-TB分离株的表型及基因型耐药谱对于一线抗结核药存在较高的一致性,但对于某些二线抗结核药其一致性较低。
