65例老年晚期肺癌的特点与预后因素分析

目的 :回顾性分析老年晚期肺癌临床及治疗特点 ,以期提高治疗水平。方法 :将 10 1例Ⅲb期与Ⅳ期老年肺癌患者分为 6 0岁～ 6 9岁组 ,70岁～ 79岁组和 5 9岁以下组。比较 3组生存率。对 6 0岁以上鳞癌组分别与腺癌组及无病理组进行比较。对 6 0岁以上Ⅲb组和Ⅳ组进行生存率比较。对 6 0岁以上综合组和单纯化疗组进行生存率比较。放疗采用60 Coγ常规分割治疗 ,总量 6 0Gy～ 70Gy ,疗程 6周～ 7周。化疗采用以顺铂为主的联合化疗方案 ,在放射治疗的前中后进行 ,最少 2周。结果 :6 0岁～ 6 9岁组和 5 9岁以下组相比 ,χ2 =2 .86 ,P >0 .0 5 ,无统计学意义。 70岁～ 79岁组和 5 9岁以下组相比 ,χ2 =1.13,P >0 .0 5 ,无统计学意义。 6 0岁以上鳞癌组和腺癌组相比 ,鳞癌组生存期较长 ,χ2 =4 .4 7,P <0 .0 5 ,有统计学意义。鳞癌组和无病理组相比 ,生存期几乎相等 ,无统计学意义。 6 0岁以上Ⅲb期组和Ⅳ组生存期相比 ,χ2 =5 .84 ,P <0 .0 1,统计学有显著意义。 6 0岁以上综合组和单纯化疗组相比 ,综合组生存期较长 ,χ2 =10 .4 3,P <0 .0 1,统计学有显著意义。结论 :对 6 0岁以上Ⅲb期组与Ⅳ期组老年肺癌和 5 9岁以下组生存率相比远期治疗疗效相似。在病理类型上 ,6 0岁以上鳞癌组的远期效果较好。在分期上?

The featore and the prognosis analyising of the elderly advanced lung cancer

Objective:To review and analyse elderly advanced lung cancer clinical and treatment feture for improve diagnosis and treatment level.Methods:101 elderly advanced lung cancer stage Ⅲb～Ⅳ divided into 60～69 years old,70～79 years old and <59 years old groups,seperately compared the survival rates,in the survival rate of the >60 years old group.The group of the squamous cell carcinoma separately compared the group of the adenocarcinoma cell carcinoma and the nopathology.The stage Ⅲb group compared the stage Ⅳ one.The chemo-radiotherapy group compared the only chemcherapy group.The radiotheraroy(RT)used 60 Coγ,given conventional fractionated DT 60 Gy～70 Gy/6w～7w.The chemotherapy was given before,during and after RT,at least 2 couses.Results:The survival rat of the 60～69 years old group compared <59 years old one,χ 2=2.86,P>0.05,the 70～79 years old group compared the <59 years old one,χ 2=1.13,P>0.05.The squamous cell carcinoma group compared the adenocarcinoma group,χ 2=4.47,P<0.05.Campared nopathology group ,almost was equal.The survival rat of the stage Ⅲb compared the stage Ⅳ,χ 2=5.84,P<0.025.The chemoradiotherapy group compared the chemothepy group,χ 2=10.43,P<0.005.Conclusions:The survival time of the >60 years old and stage Ⅲb～Ⅳ group compared the <60 years old group.The curative effect is alike.On the pathology type,The survival time of the squamous cell carcinome is more longer than the adenocarcinoma one.The stage Ⅲb was more longer than stage Ⅳ.On the treatment,the chemo-radiotherapy group obviously was superior to the chemotherapy group.To used PDD obviously raised the survival time.
