细胞黏附分子表达与子宫颈上皮内瘤变的关系

摘    要:目的:探讨细胞黏附分子表达在子宫颈上皮内瘤变(Cervical intraepithelial neoplasia CIN)中的作用。方法:应用免疫组织化学方法检测子宫颈CIN-Ⅰ30例、CIN-Ⅱ32例、CIN-Ⅲ30例中PECAM-1、ICAM-1、E-cadherin、L-选择素的表达。结果:PECAM-1在CIN-Ⅲ中的表达率明显低于CIN-Ⅰ、CIN-Ⅱ(P0.05),表达水平与子宫颈病变程度无明显相关(r=-0.2543,P0.05);L-选择素的表达率在CIN-Ⅰ、CIN-Ⅱ、CIN-Ⅲ无明显差别(P0.05),表达水平与子宫颈病变程度无明显相关(r=-0.1754,P0.05);ICAM-1在CIN-Ⅰ中的阳性表达率明显高于CIN-Ⅱ、CIN-Ⅲ的阳性表达率(P0.05),表达水平与子宫颈病变程度存在明显相关(r=-0.7864,P0.05);E-cadherin在CIN-Ⅰ中的阳性表达率明显高于CIN-Ⅱ、CIN-Ⅲ的阳性表达率(P0.05),表达水平与子宫颈病变程度存在明显相关(r=-0.8527,P0.05);ICAM-1、E-cadherin在CIN-Ⅱ、CIN-Ⅲ中的表达水平之间存在密切相关(r=0.8974,P=0.001和r=0.7853,P=0.001),其它指标表达不存在相关。结论:PECAM-1、ICAM-1、E-cadherin、L-选择素表达变化与子宫颈上皮内瘤变过程相关,其中ICAM-1、E-cadherin表达变化发挥重要作用。

Relationship between expression of cell adhesion molecule and cervical intraepithelial neoplasia
Abstract:Objective:To explore the function of cellular adhesion molecule in cervical intraepithelial neoplasia (CIN).Methods:Immunohistochemical method was used to detect the expression levels of PECAM-1, ICAM-1, E-cadherin and L-selectin in 30 cases with CIN Ⅰ, 32 cases with CIN Ⅱ and 30 cases with CIN Ⅲ.Results:The expression rate of PECAM-1 in CIN Ⅲ was significantly lower than those in CIN Ⅰ and CIN Ⅱ (P<0.05), there was no correlation between expression level of PECAM-1 and degrees of cervical lesion (γ=-0.254 3, P>0.05); there was no significant difference in expression rate of L-selectin among CIN Ⅰ, CIN Ⅱ and CIN Ⅲ (P>0.05), there was no correlation between expression level of L-selectin and degrees of cervical lesion (γ=-0.175 4, P>0.05); the positive expression rate of ICAM-1 in CIN Ⅰ was significantly higher than those in CIN Ⅱ and CIN Ⅲ (P<0.05), there was correlation between expression level of ICAM-1 and degrees of cervical lesion (γ=-0.786 4, P<0.05); the positive expression rate of E-cadherin in CIN Ⅰ was significantly higher than those in CIN Ⅱ and CIN Ⅲ (P<0.05), there was correlation between expression level of E-cadherin and degrees of cervical lesion (γ=-0.852 7,P<0.05); there was a correlation between ICAM-1 expression and E-cadherin expression in CIN Ⅱ and CIN Ⅲ (γ=0.897 4, P=0.001 and γ=0.785 3, P=0.001), there was no correlation among other indexes.Conclusion:The changes of ECAM-1, ICAM-1, E-cadherin and L-selectin expressions are related to the course of CIN, especially ICAM-1 and E-cadherin.
