子宫内膜中ER、PTEN、P16的表达及临床意义

目的:研究子宫内膜组织中ER、PTEN和P16表达,探讨其与子宫内膜癌变的关系和早期诊断及评判预后的可能性。方法:应用免疫组化S-P法对40例正常子宫内膜组织、20例子宫内膜不典型增生组织、60例内膜腺癌组织中ER、PTEN、P16的表达进行研究。结果:在正常子宫内膜、子宫内膜不典型增生、子宫内膜腺癌组织中ER、PTEN及P16的阳性表达率呈递减趋势,PTEN蛋白表达在正常子宫内膜与腺癌、不典型增生差异有统计学意义(P0.05),腺癌与不典型增生差异无统计学意义(P0.05),P16蛋白在子宫内膜腺癌与正常子宫内膜表达差异有统计学意义(P0.05),不典型增生与正常子宫内膜及腺癌差异无统计学意义(P0.05)。甾体激素受体ER表达在正常子宫内膜与不典型增生及腺癌差异有统计学意义(P0.05),ER、PTEN、P16的表达与子宫内膜癌的手术分期、组织学分级、肌层浸润有关(P0.05)。结论:ER、PTEN、P16的异常表达与子宫内膜的癌变过程相关,三者有可能成为子宫内膜组织早期癌变及评判预后的分子标记物。

Expression and clinical significance of ER,PTEN and PI6 in endometrial adenocarcinoma

Objective:To investigate alterations in expression of ER, PTEN and P16 in endometrial carcinogenesis.Methods:The expression of ER, PTEN and P16 proteins was detected by immunohistochemical staining S-P method in 40 cases of normal proliferative endometrium and 20 cases of endometrial atypical hyperplasia and 60 cases of endometrial adenocarcinoma.Results:The positive expression rates of ER, PTEN and P16 in normal proliferative endometrium, endometrial atypical hyperplasia and endometrial carcinoma were gradually decreased, there was significant difference in the expression of PTEN between normal proliferative endometrium, endometrial atypical hyperplasia and endometrial adenocarcinoma (P<0.05).There was significant difference in the expression of P16 between normal proliferative endometrium and endometrial adenocarcinoma (P<0.05), there was significant difference in the expression of ER between normal proliferative endometrial atypical hyperplasia and endometrial carcinoma. The expression of ER, PTEN, P16 protein was significantly associated with surgical stage, histologic grades and myometrial invasion of endometrial adenocarcinoma(P<0.05).Conclusion:Abnormal expression of ER, PTEN and P16 proteins is associated with carcinogenesis of endometrium. There are early events in endometria carcinogenesis and useful biomarkers for early detection and judging prognosis.
