60例极低出生体重儿临床高危因素分析

目的:探讨极低出生体重儿(VLBWI)的围产期临床特点和存活的相关因素,分析其与预后的关系。方法:回顾性分析60例VLBWI的一般情况、母孕期情况及新生儿临床特点。结果:妊高征、胎膜早破、多胎妊娠为发生VLBWI的主要因素,肺出血、呼吸窘迫综合征、颅内出血为其死亡的主要原因,呼吸暂停、感染、高胆红素血症、呼吸窘迫综合征、贫血为其常见并发症。结论:加强高危妊娠监测,预防早产,加强产科、儿科医生的合作,早发现与及时处理并发症是改善VLBWI预后的根本措施,同时要注意改善病房环境,防止院内感染的发生。

Analysis on clinical high risk factors of 60 very low birth weight infants

Objective:To explore the clinical characteristics and related factors of survive of very low birth weight infants(VLBWIs) during perinatal period,analyze the relationship with prognosis.Methods:The general conditions,maternal conditions during pregnancy and clinical characteristics of neonates were analyzed retrospectively among VLBWIs.Results:The main factors of VLBWI included hypertensive disorder complicating pregnancy(HDCP),premature rupture of membrane(PROM) and multiple pregnancy,the main causes of death were pulmonary hemorrhage,respiratory distress syndrome(RDS) and intracranial hemorrhage;the common complications of VLBWIs included apnea syndrome,infection,hyperbilirubinemia,respiratory distress syndrome and anemia.Conclusion:The monitoring of high-risk pregnancy should be strengthened,premature delivery should be prevented,the cooperation between obstetrician and pediatric doctors should be enhanced,early detection and timely treatment are fundamental measures to improve the prognosis of VLBWIs,at the same time,the ward environment should be improved to prevent the occurrence of nosocomial infection.
