宫颈刮片、阴道镜及HPV检测在宫颈癌筛查中的应用研究

目的:评价宫颈刮片、阴道镜及高危HPV检测在宫颈癌筛查中的作用,对宫颈癌筛查提出合理化建议。方法:同时进行宫颈刮片、阴道镜检查及高危HPV检测,对一项或多项异常者进行病理组织学检查,以组织学诊断作为金标准。对187例病理结果阳性(CINⅠ、CINⅡ、CINⅢ、宫颈浸润癌)的患者,比较3种方法的检出敏感度。结果:187例病理结果阳性者中,宫颈刮片阳性者114例,敏感度为60.96%;阴道镜阳性者149例,敏感度为79.68%;高危型HPV感染者155例,敏感度为82.89%,3种方法的敏感性具有统计学意义(P0.05)。结论:宫颈癌筛查及早期诊断应采用宫颈细胞学检查、阴道镜检查及HPV检测相结合,可以提高检出率,减少假阴性。

The application of PAP smear,colposcopy and HPV test in the screening of cervical cancer

Objective:To evaluate the function of PAP smear,colposcopy and high risk HPV test in screening of cervical cancer and precancerous lesions.Methods:The patients with abnormal cervical lesions were detected by PAP smear,colposcopy and high risk HPV test simultaneously.187 abnormal cases were diagnosed with pathologically,and sensitivities of them were compared with three methods.Results:114 cases were positive in PAP smear,whose sensitivity was 60.96%,whereas 149 cases were positive in colposcopy,whose sensitivity was 79.68%,HPV was positive in 155 cases whose sensitivity was 82.89%.The difference among PAP smear,HPV test and colposcopy also was significant(P<0.05).Conclusion:The combination of cytology,colposcopy and high risk HPV test can improve detective rate and accuracy,and they can decrease false negative rate.
