脑室内注入尿激酶腰穿置换脑脊液在中重度脑室出血中的应用

目的探讨中重度脑室出血的有效的治疗方法。方法将28例中重度脑室出血作为治疗组，行脑室引流注入尿激酶腰穿置换脑脊液．CT示脑室内积血基本清除后拔管；采用内科常规治疗的22例作为对照组。结果治疗组术后脑室血肿清除时间明显缩短。3个月时，治疗组死亡率为32．14％，对照组死亡率为63．64％，两组比较有显著性差异（P〈0．05）。结论脑室引流注入尿激酶腰穿置换脑脊液是一种安全有效的治疗中重度脑室出血的方法。

Treatment of Moderate-severe Intraventricular Hemorrhage by Ventricular Drainage and Intraventricular Infuse of Uroki nase and Cerebrospinal Fluid Exchange

OBJECTIVE To explore the effective treatment for moderate-severe spontaneous intraventricular hemorrhage（SIVH） . METH- ODS 28 patients with moderate-severe SIVH was treatment group, who were treated after ventricular drainage and intraventrieular in- fuse of urokinase and cerebrospinal fluid exchange. The drainage tube was with draw when the ventricle was cleaned under the CT scan. The control group was treated by only medical treatment（22 cases）. RESULTS In the treatment group, the clearence rate of hematoma increased after the operation.Three months later, the mortality rate was 32.14% in the treatment group, 63.64% in the control group, there were obvious difference in mortality rate between the two groups. CONCLUSIONS Ventricular drainage and intraventricular infuse of urokinase and cerebrospinal fluid exchange can be a safe and effective methods for moderate-severe SIVH.
