| 摘 要： | 目的探讨HPV感染,PCNA,p53的表达与子宫颈鳞状细胞癌发生、发展的相关性及临床意义。方法利用免疫组织化学的方法分别对42例子宫颈鳞状细胞癌患者进行组织学和HPV18E6、PCNA、p53的检测。结果①42例子宫颈鳞状细胞癌,角化型鳞状细胞癌30例(30/42,71.43%),未角化型鳞状细胞癌有12例(12/42,28.57%)。②HPV18E6在子宫颈鳞状细胞癌中的表达率为73.81%(31/42),在角化型鳞状细胞癌中的表达率为76.67%(23/30),角化型高于非角化型鳞状细胞癌(50.0%,6/12,P>0.05),但无统计学意义。③PCNA在子宫颈鳞状细胞癌中的阳性表达率达83.33%(35/42)。p53在子宫颈鳞状细胞癌中的阳性表达率达42.86%(18/42)。p53在有淋巴结转移的表达率76.0%(19/25)比无淋巴结转移的表达率高29.41%(5/17,P<0.05),有统计学意义。结论①HPV18E6的高表达,是预测子宫颈上皮异常增生、癌变的标志物。②HPV18E6表达与PCNA的表达呈正相关。二者的表达促进了抑癌基因p53的激活。而p53的过表达,与淋巴结的转移呈正相关,预测肿瘤恶性程度高,预后差。
