动力髋螺钉加子钉和股骨近端髓内钉治疗老年股骨转子间A2型骨折的疗效分析

目的:探讨老年股骨转子间A2型骨折的手术方式选择及疗效。方法:随机选取60例高龄患者股骨转子间A2型骨折,给予动力髋螺钉(DHS)加子钉内固定术33例和股骨近端髓内钉(PFN)内固定术27例,记录两组患者术前、术中、术后的多项相关指标并予以统计学分析。结果:所有患者均随访12~28个月至骨折愈合,平均16个月,无死亡个案;PFN组与DHS加子钉组相比,切口长度、术中出血量、手术时间、术后引流量和术后近期并发症均有显著的统计学差异(P<0.05)。结论:PFN内固定术比较适合高龄患者的骨质特点,较DHS加子钉手术创伤小,术后并发症少,较适用于老年股骨转子间A2型骨折。

To Analyze the Treatment of Intertrochanteric Fractures of Femur for Elderly Patients by Dynamic Hip Screw Combined with Screws and Proximal Femoral Nail

Objective:To compare treatment effects on elderly patients suffered from intertrochanteric fractures of the femur.Methods: We treated 60 cases of intertrochanteric fractures of the femur,33 cases were treated by dynamic hip screw combined with screws and 27 cases by PFN,all the preoperative,intraoperative and postoperative datas of the 2 groups were observed and analyzed statistically.Results: All the cases were followed up for 12 to 28 months.No one died,and all the fractures were healed.The incision length,intraoperative blood loss,operation duration,postoperative drainage and complications were statistically different between the 2 groups.Conclusion:PFN internal fixation is more suitable for the elderly patients because it has less operative trauma and fewer postoperative complications.
