肝硬化患者血清C型利钠肽前体氨基末端肽水平的变化

目的探讨肝硬化患者血清C型利钠肽前体氨基末端肽(amino-terminal pro-C-type natriuretic peptides,NT-proCNP)水平的变化及其与临床并发症的关系。方法选取65例肝硬化患者,56例肝纤维化患者和45例健康体检人群为研究对象。用ELISA方法测定血清中NT-proCNP的水平。结果肝硬化组血清NT-proCNP水平明显高于肝纤维化组和正常对照组,差异有统计学意义(P<0.01)。按肝硬化Child-Pugh’s分级,各级NT-proCNP水平无差异(P>0.05)。肝硬化患者血清NT-proCNP水平与肝腹水、静脉曲张出血和肝性脑病有关(P<0.05)。结论血清NT-proCNP水平与肝硬化的发生和发展有关,NT-proCNP水平升高可能是缓解肝硬化门脉高压的内源性代偿措施。

Changes of serum NT-proCNP in patients with liver cirrhosis

Objective To observe the changes of serum amino-terminal pro-C-type natriuretic peptides(NT-proCNP) in patients with liver cirrhosis,and to evaluate the relationship with clinical complication.Methods A total of 65 patients with liver cirrhosis,56 patients with liver fibrosis and 45 healthy controls were recruited in this study.Serum level of NT-proCNP was measured by ELISA methods.Results Serum level of NT-proCNP in liver cirrhosis group was higher significantly than that in liver fibrosis group and healthy controls(P<0.01).There was no difference according to Child-Pugh’s criteria of liver cirrhosis(P>0.05).Serum level of NT-proCNP was correlated to occurrence of ascites,variceal bleeding and hepatic encephalopathy(P<0.05).Conclusion Serum NT-proCNP may be closely associated with the progression of liver cirrhosis,and increase of NT-proCNP in patients with cirrhosis is an endogenous compensatory mechanism to reduce portal pressure.
