电子支气管镜代胸腔镜对渗出性胸腔积液的诊断价值研究

目的探讨电子支气管镜替代胸腔镜检查对渗出性胸腔积液的临床诊断价值。方法选择60例渗出性胸腔积液的患者,随机分为观察组和对照组,观察组采用电子支气管镜替代胸腔镜检查和治疗,对照组采用传统胸穿诊断和治疗,比较两组的临床效果。结果观察组30例患者中明确诊断27例,诊断阳性率90.0%(27/30),对其中19例诊断为恶性病变的患者于电子支气管镜下行喷洒药物胸膜粘连术治疗,效果满意率为94.7%(18/19);对照组中30例患者明确诊断19例,诊断阳性率为63.3%(19/30),对其中14例诊断为恶性病变的患者行常规闭式引流注药胸膜粘连术治疗,效果满意率为66.7%(8/12),两组比较差异具统计学意义(P<0.05)。结论电子支气管镜替代胸腔镜检查渗出性胸腔积液阳性率高、损伤小、操作简单,胸膜粘连术疗效好,具较高的临床应用价值,值得推广。

Research on the Diagnostic Value of Electronic Bronchoscope Replacing Thoracoscope on Pleural Exudative Effusion

Objective To observe the diagnosis value of electronic bronchoscope replaced thoracoscope on pleural exudative effusion. Methods 60 pleural exudative effusion patients were selected and divided into observation group and control group randomly. In observation group, 30 patients were observed and treated under electronic bronchoscope and in control group 30 patients were observed and treated by traditional method. The clinical effects of the two groups were compared. Results In the observation group 27 patients were diagnosed exactly, and the positive rate was 90.0%. 19 patients were diagnosed as malignant change and were treated by pleural adhesion under electronic bronchoscope. The satisfaction rate was 94.7% （I 8/19）. In the control group, 19 patients were diagnosed exactly, and the positive rate was 63.3%. 12 patients were diagnosed as malignant change and were treated by traditional method. The satisfaction rate was 66.7% （8/12）. The differences between observation group and control group were significant （P 〈0.05）. Conclusions Electronic bronchoscope replacing thoracoscope on pleural exudative effusion has higher positive rate, smaller injuries, simple operation and better therapeutic effect. It has high clinical application value and is worthy of generalization.
