动脉硬化闭塞症膝下动脉血管腔内治疗的近远期疗效观察

目的:观察动脉硬化闭塞症膝下动脉病变经皮血管腔内治疗的疗效。方法:选取我科2006年10月至2009年10月期间,动脉硬化闭塞症膝下动脉病变经皮血管腔内治疗的117例患者,回顾性分析腔内治疗开通血管的成功率及术后随访资料。结果:117例患者(132条患肢)初次腔内治疗成功率为90.15%(119/132)。术后平均随访18个月(4～36个月)。血管再狭窄发生率分别为:6个月8.5%(10/117),12个月19.0%(22/116),18个月25.9%(30/116)。21例临床症状复发,再次腔内治疗成功率85.7%(18/21)。患者手术后间歇性跛行、静息痛、缺血性溃疡等临床症状均明显改善。结论:动脉硬化闭塞症膝下动脉腔内治疗手术成功率较高,临床效果满意,远期有一定的复发率,但可再次治疗。

Short and long term outcome of patients with arteriosclerosis obliterians after infrapopliteal arterial Angioplasty

Objective：To evaluate the outcome of patients with arteriosclerosis obliterans（ASO） after percutaneous transluminal angioplasty（PTA） of the infrapopliteal arteries.Methods：117 consecutive patients with arteriosclerosis obliterans,who were demonstrated to have infrapopliteal arterial lesions by angiography and treated by PTA,were evaluated retrospectively and followed up for 3 years（from October,2006 to October,2009）.Results：117 patients（132 limbs） were treated by PTA.The primary technical success rate for PTA was 90.15%（119 of 132 limbs）.Average follow-up was 18 months（4-36 months）.The re-stenosis rates after PTA at 6,12,18 months were 8.5（10 /117）,19.0（22 /116） and 25.9%（30 /116）,respectively.21 cases appeared clinical recurrence and performed re-intervention of PTA,and the subsequent technical success rate was 85.7%（18 of 21 limbs）.The clinical symptoms including intermittent claudication,rest pain and ischemic ulcer were mitigated significantly after PTA.Conclusion：Infrapopliteal arterial PTA has a high success rate and an excellent clinical effect on patients with ASO.Although the recurrence of vascular re-stenosis may occur at a certain rate in long term,re-intervention of PTA may be performed.
