2011年重庆市碘缺乏病监测结果分析

摘    要:目的了解重庆市碘缺乏病现状和居民碘营养水平,为调整食盐加碘浓度提供科学依据。方法2011年按照“人口比例概率(PPS)抽样方法”,抽取30所小学作为监测点,每点随机抽取40名8~10岁儿童,检查甲状腺大小、尿碘水平和家中食盐碘含量;在每个监测点附近,抽取3个乡镇,每个乡镇随机捕取孕妇和哺乳期妇女各5人,检测尿碘含量。结果8—10岁儿童甲状腺检查1322人,甲肿率为5.52%;盐碘检测1321份,均数为30.46mg/kg,碘盐覆盖率、合格率、合格碘盐食用率、非碘盐率分别为99.70%、97.04%、96.74%和0.30%;尿碘检测1317人,中位数为254.1μg/L,100—199.99μg/L占23.23%,≥300p.g/L占36.52%;孕妇和哺乳期妇女尿碘共检测913人,中位数为222.5μg/L,100~199.99μg/L占30.34%,I〉300μg/L占32.20%。结论2011年重庆市儿童甲肿率较低,盐碘、尿碘指标达到国家消除标准;儿童尿碘超过适宜量,孕妇和哺乳期妇女尿碘处于适宜水平。

Abstract:Objective To investigate iodine deficiency disorders (IDD) and iodine nutrition level among the resi- dents in Chongqing for formulating preventive measures. Methods With population probability sampling method( PPS), 30 counties were selected for monitoring. Forty students aged 8 - 10 were selected from each of 30 pri- mary schools. Urinary iodine, salt iodine were detected while the thyroid content was measured by B ultrasound. Five pregnant women and 5 lactating women were randomly selected from 3 spot townships nearby and urinary io- dine of whom was tested. Results A total of 1, 322 students aged 8 - 10 were investigated and the goiter rate was 5.52%. A total sample of 1, 321 salts were test and the salt iodine was 30. 46mg/kg on average. The coverage rate, rate of qualified iodized salt, taking rate of qualified iodized salt and rate of non - qualified .iodized salt was 99.70% , 97.04% , 96. 74% and 0. 30% , respectively. The median of urine iodine in children aged 8 - 10 was 254. 1 μg/L. Urine iodine of 100 - 199.99 μg/L accounted for 23.23% and 300 μg/L of urine iodine ac- counted for 36.52% of the total. The median of urine iodine of pregnant and lactating women was 222.5p, g/L. Urine iodine of 100 - 199.99 p,g/L accounted for 30. 34% and urine iodine above 300 μg/L accounted for 32.20%. Conclusion Total goiter rate, salt iodine and urinary iodine have achieved the national standard for e- liminating IDD. The iodine nutrition of children exceed the adequate level ; iodine nutrition of pregnant and lacta- ting women is appropriate.
