养阴活血通络汤治疗类风湿性关节炎疗效及对患者关节液TNF-α、IL-1和PGE2的影响

目的: 探讨养阴活血通络汤治疗类风湿性关节炎的疗效及对患者关节液中TNF-α、IL-1、PGE₂的影响。方法:选取类风湿性关节炎患者100例,随机分为对照组和观察组各50例。对照组采取西药治疗,观察组采取西药加养阴活血通络汤治疗。比较两组治疗前后关节液TNF-α、IL-1及PGE₂水平,关节疼痛、关节肿胀及晨僵时间评分、临床疗效及生活质量情况。结果:治疗4、8、12周后,两组患者中医症状积分均明显下降,且观察组比对照组下降更明显(P<0.05)。治疗后,两组TNF-α、IL-1水平,关节疼痛、关节肿胀及晨僵时间评分均降低,PGE₂水平均升高(P<0.05); 观察组治疗后TNF-α、IL-1水平,关节疼痛、关节肿胀及晨僵时间评分均低于对照组,PGE₂水平高于对照组(P<0.05)。观察组治疗后生活质量明显优于对照组(P<0.05)。结论:养阴活血通络汤可显著缓解类风湿性关节炎患者关节疼痛、肿胀等症状,改善炎症及免疫水平,临床疗效满意,并可改善患者生活质量水平。

Effect of Yangyin Huoxue Tongluo decoction on TNF- α,IL-1 and PGE2 in the synovial fluid of patients with rheumatoid arthritis

Objective:To explore the therapeutic effect of Yangyin Huoxue Tongluo decoction on rheumatoid arthritis and its effect on TNF-α,IL-1 and PGE₂ in the joint fluid of patients.Methods:100 patients with rheumatoid arthritis were randomly divided into control group and observation group,each with 50 cases.The control group was treated with conventional western medicine,and the observation group was treated with Yangyin Huoxue Tongluo decoction on the basis of the control group.Results:After 1,2 and 4 courses of treatment,the TCM symptom scores of the two groups decreased significantly,and the observation group decreased more significantly than the control group(P<0.05).After the treatment,TNF-α,IL-1 levels,joint pain,joint swelling and morning stiffness time scores were all reduced,and PGE₂ levels were all increased(P<0.05); after the treatment,TNF-α,IL-1 levels,joint pain,joint swelling and morning stiffness time scores in the observation group were all decreased(P<0.05).The level of PGE₂ was higher than that of the control group(P<0.05).The quality of life in the observation group was significantly better than that in the control group(P<0.05).Conclusion:Yangyin Huoxue Tongluo decoction can significantly alleviate the symptoms of rheumatoid arthritis,improve the level of inflammation and immunity,the clinical effect is satisfactory.
