白虎加桂枝汤治疗痛风及对患者血清炎症因子和血尿酸水平的影响

目的: 探讨白虎加桂枝汤治疗痛风患者的疗效及对血清细胞白介素-6(IL-6)、C反应蛋白(CRP)、可溶性细胞间黏附分子-1(sICAM-1)及血尿酸(UA)水平的影响。方法:选取痛风患者100例,采用随机数字分组法分对照组和观察组,每组50例。对照组给予口服依托考昔片治疗,观察组在对照组用药基础上给予白虎加桂枝汤治疗。比较两组治疗前后血清IL-6、CRP、sICAM-1、UA水平变化、症状体征改善及生活质量情况。结果: 治疗后两组患者中医症状积分均明显下降,且观察组比对照组下降更明显,差异具有统计学意义(P<0.05)。治疗前,两组血清IL-6、CRP、sICAM-1及UA水平,关节疼痛、关节肿胀及关节活动受限程度评分比较,差异无统计学意义(P>0.05); 治疗后,两组IL-6、CRP、sICAM-1及UA水平,关节疼痛、关节肿胀及关节活动受限程度评分均降低(P<0.05); 观察组治疗后IL-6、CRP、sICAM-1及UA水平,关节疼痛、关节肿胀及关节活动受限程度评分均低于对照组(P<0.05); 观察组总有效率94.00%高于对照组80.00%(P<0.05)。观察组治疗后生活质量优于对照组(P<0.05)。结论: 白虎加桂枝汤可显著改善痛风患者临床症状,降低血清IL-6、CRP、sICAM-1等炎症因子及尿酸水平。

Effect of Baihu Jiaguizhi decoction on serum inflammatory factors and serum uric acid level in patients with gout

Objective:To investigate the therapeutic effect of Baihu Jiaguizhi decoction on gout patients and its effect on the levels of IL-6,CRP,sICAM-1 and UA.Methods:100 gout patients were selected.The control group and the observation group were randomly divided into 50 cases in each group.The control group was treated with etocoxib,and the observation group was treated with Baihu Jiaguizhi decoction on the basis of the control group.The changes of serum IL-6,CRP,sICAM-1,UA and the improvement of symptoms and signs were compared before and after treatment.Results:After treatment,the TCM symptom scores of the two groups decreased significantly,and the observation group decreased more significantly than the control group(P<0.05).After treatment,the levels of IL-6,CRP,sICAM-1 and UA,joint pain,joint swelling and joint activity limitation decreased in the two groups(P<0.05); after treatment,the levels of IL-6,CRP,sICAM-1 and UA in the observation group were decreased(P<0.05).The scores of joint pain,joint swelling and joint activity limitation in the observation group were lower than those in the control group(P<0.05),and the total effective rate in the observation group was 94.00% higher than that in the control group(P<0.05).The quality of life in the observation group was significantly better than that in the control group(P<0.05).Conclusion: Baihu Jiaguizhi decoction can significantly improve the clinical symptoms of gout patients,reduce the levels of serum IL-6,CRP,sICAM-1 and other inflammatory factors and uric acid.The clinical effect is satisfactory,and it is worthy of further promotion and application.
