食管异物致食管穿孔的外科治疗

作者对近5年10例食管异物致食管穿孔病例的临床表现、异物种类、穿孔部位及治疗过程进行了详细的分析。重点指出早期诊断、早期手术的重要性。尤其对食管中段位于主动脉弓水平的穿孔并伴有呕血的病人,要格外提高警惕,尽早手术。术前要做好体外循环、人造血管等准备;术中采用低压低温、血管置换、彻底清除病灶、抗生素冲洗胸腔等措施来确保手术的完全成功。

THE SURGICAL TREATMENT OF ESOPHAGEAL PERFORATION CAUSED BY FOREIGN BODIES

This paper presented a detailed analysis on 10 cases suffered from esophageal perforation caused by foreign objects in recent 5 years. The authors emphasized the vital importance of early diagnosis and early treatment, and pointed out that special precaution should be taken to those patients with middle esophageal perforation at the level of aortic arch accompanied by hematemesis, the operation should be done as early as possible. Well preoperative preparation includes extracorporeal circulation and artificial vessels are necessary. During the process of the operation, full measures should be taken to assure the complete success of the operation, such as lowering down the pressure and temperature, replacement of the diseased vessel together with a thorough cleaning of the infected focus, and of a cleaning wash of the thoracic cavity with antibiotic.
