发热原因不明与肺血管炎性疾病

目的:原因不明发热伴呼吸系统表现是临床常见的疑难病症之一,特别是肺血管炎性疾病,常误诊。本文回顾我院几例肺血管炎性疾病延误诊断的原因,以提高对该类疾病的警惕,争取及早诊断。方法:以回顾的方法总结我院延误诊断的几例肺血管炎性疾病病例。结果:临床表现缺乏特异性,患者发热时间长,发病后2~3个月确诊。确诊前曾用抗生素、抗结核和抗真菌等多种治疗。结论:造成延误诊断的主要原因是对该类疾病认识不足。提高对肺血管炎性疾病的认识,对于原因不明的发热伴呼吸系统表现无法用常见疾病解释,要警惕肺血管炎性疾病。

Fever of unknown origin and pulmonary vasculitis diseases

Objective:As one of the common clinical disorders,fever of unknown origin with respiratory system manifestations,especially pulmonary vasculitis diseases are often misdiagnosed.The causes that the diagnosis was delayed for several cases with pulmonary vasculitis diseases in our hospital were retrospected to promote our recognization of the diseases so as to make early diagnosis.Methods:Retrospective method was used to summarize several cases in our hospital with pulmonary vasculitis diseases which diagnosis were delayed.Results:The patients got fever for a long time and the clinical manifestations were nonspecific.Therapy such as anti-bacterial,anti-tubercle,and anti-fungus treatment was used until diagnosis was made 2 to 4 months later.Conclusion:The main reason of the misdiagnosis is the inadequate note to the diseases.It′s important to elevate our ability to know the diseases.We should consider pulmonary vasculitis diseases when met with fever of unknown origin with respiratory system manifestation which can not be explained by common diseases
