下段直肠癌双重器械吻合保肛手术49例分析

目的 总结下段直肠癌双重器械吻合保肛手术经验.方法回顾性分析49例下段直肠癌用双重器械吻合手术及术后随访资料.结果 49例中无手术死亡,术中快速活检,3例远端阳性再切除1cm以上,术后病检,直肠切除后两端均无肿瘤残留.3例盆腔感染,2例切口感染经冲洗、换药等治愈.术后均用多药联合化疗,随访1、3、5年,生存率分别100%、85.7%、61.2%.结论下段直肠癌用双重器械吻合保肛手术临床效果好.

Application of double stapling technique in anus-preserved operation for low rectal cancer

Objective To summarize the experience of use of double stapling technique in anus - preserved operation for low rectal cancer. Methods Clinical data of 49 patients with low rectal cancer who underwent anuspreserved operation（ in which double stapling technique was used for anastomosis）were retrospectively analyzed. Results there was no operation death. Based on the results of fast pathological examination ,distal ends which were found positive for cancer cells were then resected 1 cm or over in 3 cases. After the operation, the distal ends of the rectum were free of cancer cells by pathological inspection. Three patients who had pelvic cavity infection were treated by washing. Two patients who developed wound infection were treated by dressing. Post - operative multi - drug chemotherapy was carried out in all cases. The overall survival rates at 1,3 and 5 years were 100% （49 cases） ,85.7% （42 cases）and 61.2% （30 cases）. Conclusion Application of double stapling technique in anus -preserved operation is effective in the treatment of low rectal cancer.
