支气管哮喘患者治疗依从性对病情转归的影响

目的探讨患者治疗依从性对支气管哮喘(哮喘)预后的影响。方法随机选择我院2005～2010年住院及门诊哮喘患者68例,均建立医患联系卡,每周复诊,按照哮喘防治创议(G INA)方案选择长期控制相关药物进行阶梯性治疗,于治疗前及治疗3个月测定肺功能,采用呼气峰流速(PEF)和第一秒用力呼气容积(FEV1)占预计值的百分比作为观测指标。根据患者治疗依从程度分为研究组和对照组,治疗依从性好的34例纳入研究组,不规范用药的34例纳入对照组,比较两组病情转归。结果研究组没有因哮喘急性发作而急诊就医者,偶尔使用缓解药物1例(2.94%);对照组中不能坚持规范用药者28例(82.35%),其中用药剂量不准确23例(67.65%),用药方法不正确25例(73.53%),每月哮喘发作1次以上8例(23.53%),两组比较差异有统计学意义(χ2=4.61,P<0.05)。PEF%及FEV1%治疗前两组比较差异无统计学意义(P>0.05);治疗后PEF%及FEV1%均明显上升,但研究组更为明显,两组比较差异有统计学意义(P<0.01)。结论规范应用G INA方案,加强患者治疗依从性的干预,有助于提高哮喘治疗的效果。
