重度阻塞性睡眠呼吸暂停患者夜间心率变异性分析

目的 通过对重度阻塞性睡眠呼吸暂停（OSA）患者夜间心率变异性的分析，以了解OSA患者夜间睡眠时植物神经功能状态。方法 对21例重度阻塞性睡眠呼吸暂停患者及20例正常受试者分别进行夜间7小时多导睡眠呼吸监测及心率变异性分析。结果 OSA患者与对照组相比AHI，最低血氧，平均血氧，氧饱和度低于90％百分比，心率变异性指标SDNN，CV，LF／HF差异均具有显著性。且氧饱和度低于90％百分比与LF／HF呈正相关。结论 重度OSAS患者夜间存在严重低氧以及心率变异性增大，提示重度OSAS患者迷走、交感神经张力均增高且以交感神经张力增高为主。低氧的持续时间与交感神经张力呈正相关。

The analysis of heart rate variability(HRV) in 21 patients with severe obstructive sleep apnea syndrome(OSAS).

Objectives:To study HRV in patients with severe obstructive sleep apnea syndrome,find out the nocturnal variation of autonomic nervous system in OSA patients.Methods:7-hours nocturnal polysomnography and analysis of HRV were recorded in 21 patients with severe OSA and 20 controls.Results: In severe OSAS patients:AHI,the lowest oxygen desaturations(Sao 2),average Sao 2 were lower than controls,the percent of SaO 2 lower than 90% was higher than controls;SDNN,CV were higher,LF/HF was lower Positive correlation was detected between the percent of SaO 2 lower than 90% and LF/HF.Conclusion:The results suggested that the activities of sympathetic nervous and vagus nervous were enhanced in severe OSAS patients,the enhance of sympathetic nervous activity was obviously.Positive correlation between the time of hypoxaemia persistent and the activity of sympathetic nerve.
