血浆NT—proBNP、MYO、cTnI联合检测对诊断急性心肌梗死的临床意义

摘    要:目的探讨急性心肌梗死(AMI)患者血浆N端B型脑钠肽前体(NT—proBNP)、肌红蛋白(MYO)、肌钙蛋白I(cTnI)的浓度变化及三者间的相关性。方法通过酶联荧光分析法测定60例AMI患者和48例健康对照者血浆NT-proBNP、MYO、cTnI含量并进行对照分析。结果AMI组血浆NT—proBNP、MYO、cTnI含量显著高于健康对照组,差异具有统计学意义(P〈0.05);NT—proBNP与MYO、cTnI之间具有良好的正相关关系(r=0.715,r=0.805);AMI组NT—proBNP、MYO、cTnI单项及三项联合检测的敏感性分别为86.67%、81.67%、83.33%和96.67%;特异性分别为89.58%、68.75%、95.83%和68.75%。结论NT—proBNP、MYO、cTnI在AMI患者中明显升高,三项联合检测可提高AMI早期诊断的敏感性。

Abstract:Objective To study the changes in the concentrations of plasma amino-terminal pro-brain natriuretic peptide (NT- proBNP) , myoglobin (MYO) and cardiac troponin I (eTnI) in patients with acute myocardial infarction (AMI) and the correlation among the them. Methods Plasma NT-proBNP, MYO and eTnI were determined by enzyme-linked fluorescence analysis in 60 AMI patients ( AMI group) and 48 healthy controls ( control group). Results The levels of NT-proBNP, MYO and eTnI were sig- nificantly higher in AMI group than in control group ( P 〈 0.05 ). In AMI group, a significantly positive correlation was found be- tween NT-proBNP and each of MYO and eTnI ( r = 0. 715, r = 0. 805 ). The sensitivity of single NT-proBNP, MYO, cTnI and com- bined detection in AMI diagnosis was 86.67% , 81.67% , 83.33% and 96.67% , and the specificity was 89.58% , 68.75% , 95.83% and 68.75% , respectively. Conclusion The levels of NT-proBNP, MYO and eTnI are significantly higher in AMI. Combined detection helps to improve the detection sensitivity of AMI in early diagnosis.
