下颌水平及近中阻生智齿分牙拔除过程中分牙线位置及角度的选择

目的探讨下颌水平及近中阻生智齿拔除过程中分牙线位置及角度如何选择可缩短手术时间，减小手术创伤。方法选择2012年1月1日--12月31日在本院就诊需拔除下颌水平或近中阻生智齿患者105例，175颗牙，其中水平阻生77颗牙，近中阻生98颗牙，男性患者45例，女性患者60例。结果患者选择合适分牙线及分牙角度可减少拔牙手术时间，A组：（20．62±5．15）min，B组：（23．2±6．78）min（P〈0．05），并能减少术中再次分牙操作，A组：5颗，B组：18颗（P〈0．05）。结论术前评估下颌阻生智齿阻力，并于术前确定分牙线及分牙角度在下颌水平或近中阻生智齿拔除术中很有必要。

Choice of position and angle of split-line in the extraction of the mandibular horizontal and mesioangular impacted third molars

Objective To investigate how to choose the proper position and angle of the split-line in the mandibular horizontal and mesioangular impacted third molar extraction, in order to shorten the operation time and reduce surgical trauma. Methods A total of 105 patients （45 males and 60 females） including 175 mandibular impacted third molars （77 horizontal impacted molars, 98 mesioangular impacted molars） , who underwent molar extraction in6ur hospital from Jan. 1, 2012 to Dec. 31, 2012, were en- rolled in the study. Results Proper choice of position and angle of split-line helped to reduce not only the extraction operation time （20.62 ± 5.15 vs 23.2±6.78min, P 〈 0.05 ） but also the extraction number for the second time （ 5 vs 18 teeth ; P 〈 0.05 ）. Conclusion The resistance of the mandibular wisdom teeth should be assessed, and the position and the angle of the split-line should be determined preoperatively.
