嗜酸性胃肠炎临床分析

目的探讨嗜酸性胃肠炎(Eosinophilic Gastroenteritis,EG)病因、诊断、误诊原因、治疗方法.方法对我院7例嗜酸性胃肠炎临床表现、实验室检查、特殊检查结果进行分析,结合文献报道41例患者的资料,分析EG发病诱因,临床特点、内镜特征和误诊原因及治疗预后.结果EG患者以腹痛为首发症状,主要症状为腹痛、恶心、呕吐、腹泻,内镜下胃肠粘膜呈充血、糜烂为主,误诊原因:1.嗜酸性胃肠炎为临床少见病;2.临床表现多样、无特异性;3.特殊检查不能有效的提示.肌型属罕见型.皮质激素治疗疗效好,作用迅速.结论EG临床表现多样性、无特异性;易误诊,肌型属罕见型,皮质激素治疗疗效好,作用迅速.

Clinical analysis of eosinophilic gastroenteritis

Objective To investigate the aetiology, diagnosis, misdiagnosis and therapy of eosinophilic gastroenteritis.Methods Seven patients with eosinophilic gastroenteritis in our hospital from April 1989 to January 2003 were studied retrospectively. Results The main manifestations of patients with eosinophilic gastroenteritis were abdominal pain, nausea, vomiting and diarrhea. Abdominal pain was the usual clinical onset. Endoscopy always revealed mucosal erythema and erosion. Causes of misdiagnosis included. (1) Eosinophilic gastroenteritis was a rare gastrointestinal disorder. (2) Clinical presentations of eosinophilic gastroenteritis were variable. (3) Special examinations showed no character. Muscular type was uncommon. Treatment with steroid was recommended. Conclusion Symptoms of eosinophilic gastroenteritis are variable with no character. It is difficult to make the diagnosis. Muscular type is uncommon. Steroid acts quickly and leads to perfect result.
