急性冠脉综合征冠脉病变程度与血栓前状态分子标志物的相关性研究

摘    要:目的:探讨急性冠脉综合征(ACS)冠脉病变程度与血栓前状态分子标志物水平的相关性.方法:选择ACS患者32例(ACS组)和对照组21例进行选择性冠状动脉造影,对冠脉造影结果采用累计积分法进行冠状动脉狭窄评分;使用血凝分析仪测定受试者血浆纤维蛋白原(Fig)水平,使用酶联免疫法测定外周静脉血血管性血友病因子(vWF)、D-二聚体(D-D)、纤溶酶原激活剂抑制物(PAI)、血小板膜糖蛋白140(GMP-140)的含量.对2组结果进行比较,对冠状动脉狭窄评分与血栓前状态分子标志物进行Pearson相关分析.结果:ACS组Fig、vWF、D-D、PAI、GMP-140的含量均高于对照组,差异有统计学意义(P<0.05或P<0.01);Fig、vWF、D-D、PAI、GMp-140的水平与ACS冠脉病变分数、支数之间呈线性相关关系(r介于0.519~0.728之间).结论:急性冠脉综合征(ACS)外周血存在血栓前状态,且血栓前状态分子标志物水平与冠脉狭窄程度呈正相关.

Abstract:Objective: To investigate the correlation between prethrombotic state molmarker and coronary severity in acute coronary syndromes. Methods:In patients with ACS(n=32) and controls(n=21) the selective coronary arteriography was performed and the coronary artery stenosis scores were evaluated through accumulative total integration. Plasma concentrations of fibrinogen(Fig)were determined with hemagglutinin analysator and plasma concentrations of von Willebrand factor (vWF)? D-Dimer (D-D)? plasminogen activator inhibitor (PAI)?platelet membrane glucoprotein(GMP-140) were determined with enzyme linked immunosorbent assay. The results of two groups were compared and Pearson correlation analysis between coronary artery stenosis scores and prethrom botic state molmarker was done. Results:Plasma concentrations of Fig? vWF? D-D? PAI? GMP-140 were significantly higher in ACS than in controls(P<0.05 or P<0.01); Plasma concentrations of Fig, vWF,D-D,PAI, and GMP-140 were correlated with ACS and had a linear correlation to the scores and number of branches of diseased coronary vessels (r=0.519~0.728). Conclusion:Prethrombotic state exists in Peripheral blood in acute coronary syndromes(ACS) and has a positive correlation with coronary artery stenosis severity.
