摘    要:本文使用双功彩色多普勒检查急性肾小球肾炎284例,主要探查肾脏声像图改变及肾动脉多普勒血流频谱特征,以探讨肾动脉血流动力学变化的机理及搏动指数(PI)、阻力指数(RI)的临床意义。结果显示:176例双肾肿大,肾皮质回声增强。176例中110例合并心脏扩大;39例肝脏肿大;62例腹水;4例胸水。病程第一周,多普勒血流频谱呈低流速。低阻力型,第2~3周病情好转后,呈高流速,高阻力型。高血压脑病者,亦为高流速,高阻力血流频谱。本文资料提示;影响肾动脉多普勒血流频谱变化的主要因素是:①全身循环状况;②肾动脉的顺应性;③肾间质水肿对肾血管的压迫。

Abstract:This article reported the renal sonographic changes and the characters of renal artery (RA) Doppler spectrumin 284 cases of acute nephritis. The aim was to study the mechanism of the change of hemodynamics and the clinical significance of pulsatility index (PI) and resistivity index (RI) of RA. The enlarged kidney and increased renal cortex echogenicity were found in 176 cases. Among them 110 cases' heart enlarged, 39 cases with hepatomegaly, 62 cases with ascites, 6 cases with pleural effusion. The characters of Doppler spectrum of RA showed low velocity and low resistivity in first week of the disease, high velocity, high resistivity and hypertension encephalopathy in the second-third week of disease. The main factors affected the change of RA dappler spectrum were: (l ) general circulation of the patients, (2) the compliance of renal artery and (3) kidney mesenchyme edema.
