适形放疗技术在非小细胞肺癌中的临床应用探究

目的探讨三维适形放疗（3DCRT）非小细胞肺癌（NSCLC）患者的疗效和副作用。方法112例III期NSCLC患者随机分为三维适形放疗组（治疗组）和常规放疗组（常规组）各56例。治疗组予3DCRT照射，隔日1次，每次4—8Gy，总照射量40-64Gy；常规组予常规分割照射，每周治疗5次，每次1．8～2．0Gy，总照射量60。70Gy。结果3DCRT组有效率67．8％（CR＋PR），完全缓解率32．1％（CR）；常规组有效率28．6％，完全缓解率3．6％（P〈0．05）。3DRCT组1年生存率为71％，常规组为48％。3DCRT组放射性肺炎、放射性食管炎和血液反应率为21％、30％和71％，均略低于常规组。结论三维适形放疗可提高患者生存率，减轻放疗的毒副作用，治疗非小细胞肺癌比较有效。

Study on clinical application of conformal radiotherapy in treatment for non-small-cell lung cancer

Objective To investigate the results and toxic effect of three-dimensional conformal radiotherapy（3DCRT） for non-small-cell lung cancer. Methods 112 cases with stage III non-small-cell lung cancer were randomly divided into two groups; 56 in 3DRCT group and 56 in conventional group. In treatment group, patients were fractionated 3DRCT every other day and 4 - 8Gy every time. Total dosage were 40 - 70Gy.Results In 3DRCT group, the overall response rate was 67.8%（CR＋PR）and complete remission rate was 32.1%（CR） while conventional group was 28.6% and 3.6%. respectively 3DRCT group was higher（P〈 0.05）.Conclusion Three-dimensional conformal radiotherapy can increase the survival rate of patients and ease the toxic effect of chemotherapy. In a word, it＇ s an effective way to treat non- small-cell lung cancer.
