晨尿蛋白/尿肌酐与24h尿蛋白定量相关性分析

摘    要:目的探讨晨尿蛋白/尿肌酐(P/Cr)与24h尿蛋白定量之间的相关性。方法选取261例住院患者,共510份标本,对其晨尿P/Cr与24h尿蛋白定量进行相关性分析。采用ROC曲线分析晨尿P/Cr相对于24h尿蛋白定量3.5、1.0和0.5g的诊断界点。结果住院患者晨尿P/Cr与24h尿蛋白定量呈中度相关。按血清白蛋白<30g/L和≥30g/L分组,进一步分析显示:当血清白蛋白<30g/L时晨尿P/Cr与24h尿蛋白定量呈高度相关;当血清白蛋白≥30g/L时晨尿尿蛋白/尿肌酐比值与24h尿蛋白定量亦呈中度相关。应用ROC曲线分析晨尿P/Cr相对于24h尿蛋白定量3.5、1.0和0.5g的诊断界点分别为2.97、0.93和0.64g/gcr时敏感性和特异性最佳。结论监测尿蛋白排出情况时,患者晨尿P/Cr可以替代24h尿蛋白定量。

The Correlation Between First Morning Urine Protein-creatinine Ratio and 24-hour Urine Protein Excretion

Abstract:Objective To evaluate the correlation between 24 hours total urine protei and urine protein-to-creatinine ratio in first morning urine specimens.Methods 510 aliquot of 24-hour urine collections and first morning urine specimens from 261 inpatients were collected,The correlations between P/Cr in first morning urine specimens and urinary protein excretion in 24-hour collections were analyzed.The cutoff values of P/Cr in first morning urine specimens for screening urinary protein excretion of 3.5g,l.0g and 0.5g in 24-hour collections were determined by receiver operating characteristics(ROC)curve.Results There was a moderate significant correlation between the urine protein excretion in 24-hour collections and the P/Cr in first morning urine specimens.Further analysis showed that the correlation was highly significant for patients with Serum Albumin<30g/L.And for patients with Serum Albumin≥30g/L,the correlation was a moderate significant.By ROC curve analysis,the P/Cr of 2.97g/gcr,0.93g/gcr and 0.64g/gcr in first morning urine specimens represented the most appropriate threshold to detect urine protein excretion of 3.5g,1.0 and 0.5g in 24-hour collections,respectively.Conclusions The P/Cr in first morning urine samples can used as an alternative to urine protein excretion in 24-hour collections to monitor the excretion of urine protein in the patients with renal diseases.
