剖宫产术后子宫切口血肿20例分析

目的探讨剖宫产术后子宫切口血肿形成的原因、诊断及防治措施。方法回顾性分析2004年5月—2008年12月我院收治的20例剖宫产术后子宫切口血肿患者的临床资料。结果 20例患者均在术后1周~2周左右B超检查发现子宫切口处低回声或无回声为主的混合性团块,提示子宫切口血肿。其中16例患者经抗感染、促宫缩并辅以中药活血化瘀等治疗后痊愈,3例患者行子宫切除术,1例患者行子宫动脉栓塞术。结论剖宫产术后子宫切口血肿形成原因有多种,B超检查能早期诊断,早期合理应用抗生素并辅以中药治疗及支持对症治疗,是保守治疗成功的关键。

Analysis of 20 cases of uterine incision hematoma after cesareansection

Objective To investigate the causes,diagnosis,prevention and treatment of the uterine incision hematoma after cesarean section.Methods A retrospective review of 20 cases of uterine incision hematoma after cesarean section in our hospital from May 2004 to December 2008 were made.Results After about 1-2 weeks,B-ultrasonography examination revealed hypoechoic or no echo-based mixed echo mass in the uterine incision,suggesting that uterine incision hematoma.16 cases were undertaken anti-infection and promoting uterine contraction and promoting blood circulation by removing blood stasis of Chinese traditional medicine.3 cases were administered with hysterectomy.1 cases was administered with uterine artery embolization.Conclusion There are many causes of uterine incision hematoma after cesareansection.Early diagnosis by B-ultrasonography,early rational use of antibiotics and supplemented with Chinese traditional medicine treatment and support for symptomatic treatment is the key to success.
