摘    要:目的:评价多层螺旋CT血管成像(multi-slice computed tomographic angiography,MSCTA)在支气管扩张并大咯血患者介入治疗中的作用。方法:16例支气管扩张并大咯血的患者,采用16层螺旋CT增强薄层扫描,经最大密度投影(MIP)及容积重建(VR)处理显示罪犯血管,然后行支气管动脉栓塞(bronchial artery embolization,BAE)治疗。结果:16例患者胸部CT血管成像共显示28条罪犯血管,包括21条支气管动脉(左13、右8)、3条肋间动脉、2条膈动脉、1条胸廓内动脉及1条自主动脉弓部下缘来源。栓塞后随访6—13个月(平均9个月),15例(93.7%)未发生再咯血。结论:MSCrA能客观地评价咯血相关血管特征,指导支气管扩张并大咯血的介入治疗。

Abstract:Objective:To evaluate the utility of multi-slice computed tomographie angiography (MSCTA) in interventional therapy of the bronehiectasis with massive hemoptysls. Methods: Thin-section enhanced 16-slice CT scanning was performed on 16 patients of bronehieetasis with massive hemoptysis before bronchial artery embolization(BAE). Post processing technique, i. e, maximum intensity projection(MIP) and volume rendering (SVR), were used to display the fending arteries. Results: 16-slice CT angiography clearly demonstrated 28 focus arteries, including 21 bronchial arteries(13 left and 8 right), 3 intercostal arteries, 2 inferior phrenie arteries, 1 r internal thoracic artery, and 1 variation branch of aortic arch. Long-term control of bleeding was achieved in 15 (93.7%) followed up 6 to 13 months (median follow- up of 9 months ) after BAE. Conclusion: MSCTA can reveal the fending vessels objectively and serve instruction of inervention and therapy in the bronehiectasis with massive hemoptysis.
