提高糖尿病胰岛素疗效和顺应性及健康教育的对策研究

目的探讨提高糖尿病(DM)患者胰岛素治疗顺应性和疗效的临床对策。方法问卷调查影响胰岛素临床应用的因素并对患者进行系统的糖尿病教育。将120例住院患者按胰岛素应用方式和应用胰岛素的种类分组,比较评价胰岛素治疗的疗效和顺应性。结果系统的糖尿病教育后,患者对胰岛素治疗相关问题的知晓率均由15%以下提高到60%以上。胰岛素注射用笔组(简称胰岛素笔组)、胰岛素泵组与普通注射针组、B-D针组分别比较,其不良反应的发生率均明显降低(分别P<0.05,P<0.01),坚持应用胰岛素超过半年者明显增多(P<0.01)。胰岛素泵组治疗达标率较其它三组明显增高(P<0.01),胰岛素日平均量均显著减少;胰岛素笔组平均月费用较其它三组明显降低(P<0.01),胰岛素泵组平均月费用与普通注射针组、B-D针组分别比较均无统计学差异(P>0.05)。结论系统的糖尿病教育是提高胰岛素治疗顺应性和疗效的前提;应用胰岛素泵和诺和笔是提高治疗疗效和顺应性的有效方式。
