胸腔积液对患者肺功能影响的研究

目的探讨胸腔积液患者肺功能的变化情况。方法将90例胸腔积液患者分为少量胸腔积液组、中量胸腔积液组、大量胸腔积液组,另设30例非吸烟健康者作为对照组,对其肺功能进行前瞻性的研究。结果各胸腔积液组VC%、FVC%、TLC%、VC25%、VC50%、DL-CO%、DLCO/VA%、VA%均有所降低,与对照组比较有显著性差异(P<0.001),而且随着胸水量的增加逐渐降低;中量和大量胸腔积液组RV比对照组和少量胸腔积液组增高(P<0.001);各胸腔积液组RV/TLC均比对照组高(P<0.001)。结论胸腔积液对肺功能具有一定的影响,少量胸腔积液对肺功能的影响很小,大量胸腔积液对肺功能的影响却很严重。

Study of the effect of hydrothorax on lung function in the patients

Objective It is to explore the changes of lung function of patients with hydrothorax.Methods 90 cases of patients with hydrothorax were respectively divided into hydrothorax groups of small quantity,mild quantity and great quantity.30 cases of healthy persons with no smoking were chosen as control group.Prospective study of lung functions in all the groups were performed on.Results VC%,FVC%,TLC%,VC25%,VC50%,DLCO%,DLCO/VA%,VA% all decreased in the hydrothorax groups,and compared with those in control group,the difference was significant(P<0.001).When the quantity of hydrothorax increased more,the lowing degrees of the indexes above were lower.RV in hydrothorax groups of mild and great quantity was higher than that in control group and small quantity group(P<0.001).RV/TLC in every hydrothorax group was higher than that in control group(P<0.05).Conclusion Hydrothorax has some effect on lung function.The effect of small quantity of hydrothorax is slight,and that of great quantity is serious.
