交锁钉与钢板治疗肱骨干骨折的疗效比较

目的：研究肱骨交锁髓内钉治疗肱骨干骨折的临床效果，并评价其与钢板螺钉内固定相比较。方法：选取2003年采用肱骨交锁髓内钉治疗的肱骨干骨折56例，与1999年38例采用钢板螺钉内固定治疗肱骨干骨折的住院时间、骨折愈合时间、功能恢复效果相比较。两组患者的骨折特征和人口统计学特征无明显差异。结果：交锁钉内固定治疗组患者住院时间(9．91 3．4)d，骨折愈合时间(74．4 7．2)d，均少于钢板螺钉内固定治疗组(12．1 2．7)d。(108．2 8．7)d。采用Rodriquez—Merchan标准评价疗效，两组患者有明显差异。结论：与钢板螺钉内固定相比，肱骨交锁髓内钉对组织破坏小，术后功能恢复好；稍高的器械费用可以通过缩短骨折愈合时间等从经济上来弥补。肱骨交锁髓内钉更适合治疗肱骨干骨折。

Treatment of Humeral Shaft Fracture with Locked Intramedullary Nails and Comparison with Plates

Objective: Objective: To study the experience of Locked Intramedullary Nails in the treatment of humeral shaft fracture and to evaluate the superior clinic effecs with plates. Methods: Data were collected on 56 humeral shaft fractures treated with Locked Intramedullary Nails in 2003, and compared with retrospective data in 1999 on 38 humeral shaft fractures with Plates. The patients demographics and fracture characteristics of the two treatment groups are not significantly different in statistics. Results: The Locked Intramedullary Nails group had significantly shorter hospitalization time and time to union than the plates group, which were(9.91+3.4)d versus (12.1+2.7)d and (74.4+7.2)d versus (108.2+8.7)d respectively. The evaluation to function recovery by Rodriquez-Merchan Standard between Locked Intramedullary Nails group and plates group was still significantly different statistically. Conclusion: Humeral Locked Intramedullary Nails offered a more satisfactory and less invasive treatment effects than plates in the treatment of humeral shaft fracture. To patients, the higher cost of Locked Intramedullary Nail might be compensated by shortening the time to union. Locked Intramedullary Nails is more suitable than plates for humeral shaft fracture.
