摘    要:目的探讨MR的2D FIESTA序列在肝门区胆管癌的应用价值.方法回顾性分析18例经手术病理证实的肝门部胆管癌的2D FIESTA的表现,并与其他序列MR图像对比及与手术进行对照.结果2D FIESTA显示肿瘤瘤体18例,与胆管的关系均显示清楚,局限于胆管内肿块1例,沿胆管壁生长者4例,形成胆管周围肿块13例;胆管局限性狭窄4例,胆管完全梗阻14例;门脉左支受侵3例,门脉右支受侵4例,门脉主干受侵7例;肝动脉受侵1例.结论FIESTA作为一种快速成像序列,能清楚显示肝门部胆管癌与周围胆管及血管的关系,对肝门部胆管癌做出初步术前诊断,在评价胆管与血管的病变方面有巨大的潜能.

Abstract:Objective To evaluate the application value of MR 2D FIESTA sequence in hilar cholangiocarcinoma. Methods The imaging features of 18 cases of hilar cholangiocarcinoma on 2D FIESTA were analysed respectively, and were compared with images of other MR sequence and surgical findings. Results On 2D FIESTA, all 18 cases were demonstrated. Of the 18 cases, 1 case limited in hepatic biliary, 4 cases grew along the wall of biliary tract, 13 cases formed masses around biliary tract, 4 cases were of narrow biliary tract, 14 cases were of completely obstructed biliary tract, 3 cases involved the left portal vein, 5 cases infiltrated the right of portal vein, 7 cases involved the main portal vein, 1 case involved hepatic artery. Conclusion As a fast imaging sequence, FIESTA can show the relationship of hilar cholangiocarcinoma and biliary tract and vessels, and have tremendous latent ability on evaluating diseases of biliary tract and vessel.
