医护一体化会诊管理对老年髋部骨折手术时机的影响

目的 探讨医护一体化会诊管理对老年髋部骨折手术时机的影响.方法 选取老年髋部骨折手术患者共60例,随机分为观察组30例和对照组30例.观察组采用医护一体化理念,医生开具会诊医嘱后第一时间通知责任护士,由责任护士联系并与会诊医生进行沟通,根据会诊情况可再次电话沟通.会诊后第一时间通知主管医生或者值班医生,做到早期执行会诊意见.对照组执行原有模式.记录两组会诊时间、会诊执行时间以及入院后手术实施时间.结果 观察组会诊时间(4.13±1.59)h、会诊执行时间(1.67 ±0.80)h、入院后手术实施时间为(2.73 ±0.83)d;对照组会诊时间(14.97±6.59)h、会诊执行时间(3.40±2.19)h、入院后手术实施时间为(4.07±1.05)d;经统计学分析,两组在会诊时间、执行时间、手术实施时间均差异有统计学意义(P＜0.001).结论 通过医护一体化的实施可有效缩短老年髋部骨折的会诊时间及其执行时间,可以早期开展手术、有效缩短住院时间,减少并发症,获得较好的预后.

The effect of integrated nursing consultation management on the timing of operation for hip fracture in elderly patients

Objective With through extended responsibility nursing concept in the elderly hip fracture patients,doctors and nurses do it together,actively participating in the consultation behavior of patients,to ensure early consultation,is conducive to the early to carry out surgical treatment,it can effectively shorten the hospitalization days. Methods Doctors and nurses working close as a team,After the doctor issued a consultation notice,telephone the responsibility of the nurse immediately,then the nurse contact with the doctor who taking the consultation,according to the consultation can call again.After consultation,notify the competent doctor or duty doctor the first time,so early implementation of the consultation advice. Results The consultation time was(4.13±1.59) h,the consultation time was(1.67±0.80) h,the execution time of operation was(2.73±0.83) d in the study group;the consultation time was(14.97±6.59) h,the consultation execution time of operative time was(3.40±2.19) h,the execution time of operation was (4.07±1.05) d in the control group;by statistical analysis,There were statistically significant in the time of consultation,implementation the consultation and operation between the study group and the control group (P<0.001). Conclusion Through the implementation of health team integration can effectively shorten the time of consultation in elderly hip fracture and its execution time,that can get up early to carry out the operation,shorten the hospitalization time,reduce complications and achieve good prognosis.
