玉屏风颗粒对急性发作期哮喘患儿H4R表达的影响

摘    要:目的 观察哮喘患儿组胺H4R和人白细胞介素-12(P70)[IL-12(P70)]的表达水平及玉屏风颗粒对哮喘患儿组胺H4R和IL-12(P70)的表达影响.方法 支气管哮喘急性发作期患儿、支气管哮喘临床缓解期患儿及健康体检儿童各60例,检测外周血单个核细胞中组胺H4R mRNA和蛋白表达及血清IL-12(P70)的水平.将60例支气管哮喘急性发作期患儿随机分为两组,其中常规治疗组30例,服用孟鲁司特8周;玉屏风组30例,除给予孟鲁斯特治疗外,联合玉屏风颗粒口服治疗8周,检测玉屏风组和常规治疗组治疗前后组胺H4R mRNA和蛋白表达变化及血清IL-12(P70)水平的变化.结果 哮喘急性发作期组患儿单个核细胞中H4R mRNA和蛋白的表达明显高于哮喘临床缓解期组患儿、健康对照组儿童(P<0.05),哮喘急性发作期组患儿血清IL-12(P70)低于哮喘临床缓解期组患儿、健康对照组儿童(P<0.05),但三者哮喘临床缓解期组、健康对照组之间表达量差异无统计学意义(P>0.05).玉屏风组治疗前后H4R的mRNA及血清IL-12(P70)水平变化差异有统计学意义(P<0.05);常规治疗组治疗前后H4R的mRNA及血清IL-12(P70)水平变化差异无统计学意义(P>0.05);玉屏风组治疗后H4R的mRNA水平、血清IL-12(P70)水平与常规治疗组治疗后的水平变化差异有统计学意义(P<0.05).哮喘患儿玉屏风组治疗前后H4R的蛋白表达变化有差异,常规治疗组治疗前后H4R的蛋白表达无明显差异.结论 组胺H4R和IL-12(P70)参与支气管哮喘的发病过程,玉屏风颗粒可减少哮喘患儿H4R的表达,提升IL-12(P70)的水平,可能是玉屏风颗粒免疫调节机制之一.

Abstract:Objective o observe the expression of histamine H4R and IL-12 (P70)in children with asthma and the effect of Yuping-feng granules on the expression of H4R and IL-12 (P70)in asthmatic children.Methods Sixty children with bronchial asthma during acute exacerbation,60 children with bronchial asthma during clinical remission and 60 healthy children were enrolled in the study.The levels of H4R mRNA and protein in peripheral blood mononuclear cells and serum IL-12 (P70)of three groups were detected.To de-tect the expression of H4R mRNA and protein in peripheral blood mononuclear cells and the levels of serum IL-12 (P70)of these three groups.60 cases of acute exacerbation of bronchial asthma were randomly divided into two groups,including 30 cases of control group, taking montelukast for 8 weeks;treatment group of 30 cases,in addition to the treatment of montelukast,combined with Yupingfeng gran-ules for 8 weeks.The changes of histamine H4R mRNA and protein expression and serum IL-12 (P70)levels in Yupingfeng group and control group were detected before and after treatment.Results The expression of H4R mRNA and protein in peripheral blood mono-nuclear cells of children with acute exacerbation of asthma group were higher than that in asthma group with clinical remission and healthy children group (P <0.05),The level of serum IL-12 (P70)in children with acute exacerbation of asthma group was signifi-cantly lower than that in children with clinical remission of asthma group and healthy control group (P <0.05),but the three of them in children with clinical remission of asthma group were no significant difference between healthy children group (P >0.05).The level of H4R mRNA and serum IL-12 (P70)before and after treatment in treatment groups were statistically different (P <0.05).There were no significant differences in the level of H4R mRNA and serum IL-12 (P70)between the control groups before and after treatment (P >0.05).The level of H4R mRNA and the serum IL-12 (P70)level of treatment group were significantly different from that of con-trol group after treatment (P <0.05).The expression of H4R protein in children with asthma before and after the treatment in treatment groups wsa different,but the expression of H4R protein before and after treatment in the control groups was not significantly different. Conclusion Histamine H4R and serum IL-12 (P70)may be involved in the pathogenesis of bronchial asthma.Yupingfeng granules could reduce the expression of histamine H4R and improve the level of serum IL-12 (P70).Yupingfeng granules may regulate the level of IL-12 (P70)by regulating the expression of histamine H4R,which can regulate the immune function of asthma.
