吸烟相关性肺朗格汉斯细胞组织细胞增生症病例报告和文献复习

目的 观察吸烟相关性肺朗格汉斯细胞组织细胞增生症(pulmonary Langerhans cell histiocytosis,PLCH)病理学、免疫组织化学特征及其影像学的动态变化.并复习有关PLCH的诊治进展.方法 1例经开胸肺组织活检、免疫组织化学和高分辨率CT(HRCT)证实的吸烟相关性PLCH患者,动态观察戒烟1年后肺HRCT影像学改变.结果 患者有吸烟史(吸烟指数200),确诊后经戒烟1年后肺HRCT显示结节性和囊性病变完全消失.结论 吸烟是本例PLCH患者发病主要原因,戒烟是治疗吸烟相关性PLCH的主要方法.

Report of smoking related pulmonary Langerhans cell histiocytois and literature review

Objective To observe the pathology,imunochemestry and high-resolution computed tomography(HRCT)of smoking related pulmonary Langerhans cell histiocytosis(PLCH).Methods One case of PLCH,proved by Iung HRCT,lung biopsy and imunochemestry was described.The diagnosis and therapy of this disease was discussed,in detail,against the background of the literature.Results The patient had a smoking history(smoking index 200).Following smoking cessation,lung nodules and cysts gradually disappeared on serial computed tomography scans,with complete clearance of the lesions Mter one year.Conclusions There is close temporal relationship between smoking cessation and radiologic improvement in this patient.Smoking cessation is essential and basic therapy for the smoking related PLCH.
