无金属残留腹腔镜阑尾切除术疗效评价

目的 :评价无金属残留腹腔镜阑尾切除术的疗效。方法 :比较无金属残留 (超声刀 +圈套器 )腹腔镜阑尾切除术、传统 (电刀 +钛夹 )腹腔镜阑尾切除术和开腹阑尾切除术的手术时间、术后恢复各项指标及并发症。结果 :无金属残留腹腔镜阑尾切除术组手术时间、肛门排气时间较其它两组明显缩短 (P <0 0 5 ,P<0 0 1) ;术后镇痛剂应用、住院时间、切口感染率、粘连性肠梗阻发生率和腹腔盆腔脓肿发生率等指标明显优于开腹阑尾切除术组 (P <0 0 1) ,与传统腹腔镜阑尾切除术组无明显差异 (P >0 0 5 )。结论 :无金属残留腹腔镜阑尾切除术明显好于传统腹腔镜阑尾切除术和开腹阑尾切除术。

Evaluation of laparoscopic appendectomy without metal left

Objective:To evaluate the effect of laparoscopic appendectomy (LA)without metal left using harmonic scalpel and endoscope.Methods:Operation time and postoperative recovery parameters and complication of LA without metal left were compared with that of traditional LA using electrome and clips and open appendectomy.Results:Operation time and passage of gas by anus time in LA without metal left was shorter than those in tranditional LA or open appendectomy(P<0 05,P<0 01).There were significant difference between LA without metal left and open appendectomy (P<0.01),but no significant difference between LA without metal left and traditional LA (P>0.05)in following parameters:postoperative administration of analgesia,postoperative hospitalization,the infectious of incision,the incidence of postoperative adherent intestinal obstruction and the incidence rate of abdominal or pelvic cavity abscess. Conclusions:LA without metal left has more advantages than traditional LA and open appendectomy.
