痔上动脉结扎并外剥内扎术治疗多发混合痔临床观察

目的：研究动脉结扎术和外剥内扎术双术并行治疗混合痔的效果。方法：回顾性分析本院经过手术治疗的120例多发混合痔患者的资料，以痔上动脉结扎并外剥内扎术治疗的患者为观察组（n=60），以外剥内扎术治疗的患者为对照组（n=60），比较两组的手术效果。结果：观察组的疼痛时间为（3.1±0.9）d，愈合时间为（11.2±1.9）d，大出血3例（5.0%），排便困难5例（8.3%），肛缘水肿10例（16.6%），肛门狭窄0例。观察组在术前、术后即刻、1 d、3 d和1周的VAS评分分别为（7.5±1.9）分、（3.0±1.8）分、（3.1±2.3）分、（3.5±2.4）和（3.0±2.5）分。观察组的疼痛时间、愈合时间、疼痛评分（术后3 d、1周）及并发症例数均明显高于对照组，差异均有统计学意义（P〈0.05）。结论：痔上动脉结扎并外剥内扎术治疗多发混合痔的疗效优于单行外剥内扎术的传统方法，值得临床借鉴。

Clinical Observation of Hemorrhoid Artery Ligation and Milligan-Morgan in the Treatment of Multiple ;Mixed Hemorrhoids

Objective：To study the effect of hemorrhoid artery ligation and Milligan-Morgan in the treatment of multiple mixed hemorrhoids.Method：120 patients with multiple mixed hemorrhoid in our hospital after surgical treatment were retrospectively analyzed,the patients treated with hemorrhoid artery ligation and Milligan-Morgan therapy were set up the observation group（n=60）,the patients treated with Milligan-Morgan therapy were set up the control group（n=60）, the difference of the operation effect between the two groups were compareed.Result：In the observation group,the pain of time was（3.1±0.9）days,healing time was（11.2±1.9）days,the number of bleeding were three cases（5.0%）, the number of bowel movements difficult were five cases（8.3%）,the number of anal margin edema were 10 cases （16.6%）,the anal stenosis had no one case,accounting for 0.0%. The patients in the preoperative,VAS scores immediate postoperative,1 day,3 days and 1 week were（7.5±1.9）,（3.0±1.8）,（3.1±2.3）,（3.5±2.4）and（3.0±2.5）. The pain of time,the healing time,the number of complications,pain scores（3 d,1 week after operation）of the observation group were significantly higher,there are significant differences between the two group were compared,the differences were statistically significant（P〈0.05）.Conclusion：The curative effect of hemorrhoid artery ligation and Milligan-Morgan in the treatment of multiple mixed hemorrhoids is better than that of Milligan-Morgan,is worthy of reference for clinical.
